Objective:1.To assess the general situation of patients with different types of atrial fibrillation(AF)by observing and analyzing the clinical laboratory parameters of different types of patients with AF.2.To investigate the relationship between plasma homocysteine(Hcy),high density lipoprotein(HDL)and left atrium diameter(LAD)in patients with AF.Methods:A retrospective collection of 134 patients with atrial fibrillation who were hospitalized at the Second Clinical Medical College of Shanxi Medical University from January 2013 to 2017 was selected as the guideline for treatment of atrial fibrillation according to 2012 ESC Atrial fibrillation will be selected according to the duration of the patients were divided into.There were 63 cases of paroxysmal atrial fibrillation lasting less than 7 days,54 cases of persistent atrial fibrillation lasting more than 7 days and less than 1 year,and 17 cases of persistent atrial fibrillation lasting 1 year.54 patients who were hospitalized for sinus rhythm in our hospital during the same period were selected as control group.Patients were collected for age,gender,past history,current medical history,smoking history and history of drinking,and medication taking,including folic acid,vitamin B12 and niacin.Statistics included subjects BMI,SBP,FPG and EF.The general information of each group was compared with Hcy,HDL and LAD to analyze the relationship between Hcy,HDL and LAD.Results:1.Patients in the paroxysmal atrial fibrillation group,persistent atrial fibrillation group and permanent atrial fibrillation group were compared with the control group in the age(69.43 ± 10.50 years,71.06 ± 11.19 years,72.81 ± 6.57 years,69.96 ± 7.27years),smoking(31.74%,24.07%,23.53%,37.04%),alcohol consumption(11.11%,12.96%,11.76%,9.26%),Diabetes mellitus(20.63%,14.81%,17.65%,12.96%),coronary heart disease(17.46%,33.33%,29.41%,37.04%),body mass index(24.63 ± 3.45kg/m2,25.60±4.44kg/m2,23.69±3.88 kg/m2,24.34±3.49 kg/m2),blood pressure(132.18±18.63 mm Hg,133.50±19.14 mm Hg,129.69±17.28 mm Hg,132.54±17.81 mm Hg),fasting blood glucose(5.70±1.69 mmol / L,5.53±1.34mmol/L,5.26±0.92 mmol/L,5.55±1.38 mmol/L)and total cholesterol(4.01 ± 0.49 mmol / L,4.02 ± 1.02 mmol / L,3.84 ± 0.97 mmol / L,4.20± 0.97 mmol / L).There was no significant difference between the two groups(P> 0.05).The left ventricular ejection fraction(61.79 ± 5.88%,60.93 ± 10.18%,60.20 ± 6.86%)in AF group was significantly lower than that in control group(66.39 ± 6.91%)(P <0.05)The difference was not statistically significant(P > 0.05).2.In the control group(14.90±6.91umol/L),paroxysmal atrial fibrillation group(20.43 ± 7.54 umol/L),persistent atrial fibrillation group(29.52± 16.81umol/L)and permanent atrial fibrillation group(37.20 ± 23.39 umol/L),the levels of Hcy increased successively,the difference was statistically significant(t=2.36,6.01,6.20,P<0.05).For the LAD levels,the atrial fibrillation groups were higher than the control group(32.38 ±3.85mm)(t=3.76,8.76,4.89,P < 0.001),while the permanent atrial fibrillation group(41.0 ± 3.93mm)and the persistent atrial fibrillation group(42.90±5.43mm)had the highest level,but there was no significant difference between the two groups(t=1.08,P> 0.05).Comparison of HDL levels,the persistent atrial fibrillation group(1.17 ±0.30mmol/L)and permanent atrial fibrillation group(1.16 ± 0.25mmol/L)were significantly lower than that of paroxysmal atrial fibrillation group(1.29 ± 3.06mmol/L)and the control group(1.22 ± 0.33mmol/L)(t=2.09,2.00 and t=2.59,1.98,P<0.05),but the paroxysmal atrial fibrillation group compared with control group,permanent atrial fibrillation group and persistent atrial fibrillation group had no significant difference(t=1.32,0.20,P>0.05).There was a positive correlation between Hcy and LAD in atrialfibrillation group(r=0.56,P<0.001),and in atrial fibrillation group,HDL and LAD were negatively correlated(r =-0.29,P < 0.05).Conclusion:1.Plasma Hcy levels are closely related to LAD in patients with atrial fibrillation and may be involved in the pathogenesis and progression of atrial fibrillation.2.HDL levels associated with LAD,the reduction may be related to the progression of atrial fibrillation.
