| Dry eye is a general term for a variety of diseases,which refers to the abnormal quality or abnormal quantity or abnormal tear dynamics caused by any reason,resulting in a decline of tear film stability,and accompanied by eye discomfort and(or)any ocular surface tissue lesions.In clinical department of dermatology,connective tissue disease such as systemic lupus erythematosus(SLE),sermatomyositis and scleroderma,ften combine with symptoms of dry eye.Some of patients do not meet the diagnostic criteria for Sjogren’s syndrome.Besides,Patients diagnosed with some erythematous scaly disease or some sterile pustulosis,and long time treated with Systematic Retinoids can also dvvelop some symptoms like dry eye,dry month,dry vagina and other mucosal dryness in different degree.Clinically,when we treat the orignal diseases we mentioned previously,we often give more local symptomatic treatment to treat the dryness,such as artificial tears,non-steroidal anti-inflammatory drugs,moisturizing lubricant,etc.But the effect is not ideal.Dry eye in Traditional Chinese medicine(TCM)belongs to the category of "Syndrome of shen shui jiang ku","Disease of bai se","Dry syndrome" and so on.Its pathogenesis is deficiency of liver and kidney,deficiency of body fluid,leading to the ocular nourishmen deficiency.Zengye Decoction,wich is earliest recorded in the book of "Wen bing tiao bian",mainly treat Yin deficiency syndrome.Objective: In the study,Decoction of Zengye is applied to treat the patients with common dry eye syndrome in department of dermatology,which is based on Traditional Chinese medicine theory-“same therapeutic method can be used to treat diferent diseases”,“overall concept ” and “syndrome differentiation”.Method: Fifty-four patients(108 eyes)with dry eye accord to dry eye diagnostic criteria in the department of dermatology of The First Affiliated Hospital of Dalian Medical University from 2014 to 2016.Patients were divided into two groups randormly: treatment group(28 cases),treated with modified Zengye Decoction orally combined with Sodium Hyaluronate eye drops regularly;control group(26 cases),treated with pure Sodium Hyaluronate eye drops regularly.The course of treatment is one month.One day before and after the research,subjective symptom score,schirmer text(SIT)and tear break-up time(BUT)of the two groups were observed.Result: 1.Before and after the treatment,the improvement of average symptom score,average break-up time of tear film and average tear secretion in each group:After treatment,the average symptom score of the control group decreased(P< 0.0001),and at the same time the average BUT and SIT of the left eye and right eye increased(the left eye’s SIT,P= 0.001,the rest of indicators,P< 0.0001).It indicates that before and after the treatment the indicators of the control group have improved.On the other hand,after treatment the average symptom score of the treatment group obviously decreased(P< 0.0001),and at the same time the average BUT and SIT of the left eye and right eye obviously increased(P<0.0001).It indicates that before and after the treatment the indicators of treatment group have obviously improved.2.After treatment,the changes of average symptom score,two objective indexes and the total effective rate between two groups : After treatment,symptom score of the treatment group was reduced by an average of 2.92 point,and is significantly higher than the control group reduced by an average of 1.39 point;After treatment,BUT of left eye of the treatment group was increased by an average of 3.50 seconds,and is significantly higher than the control group increased by anaverage of 1.43 seconds;The BUT of right eye of the treatment group was increased by an average of 3.19 seconds,and is significantly higher than the control group increased by an average of 1.43 seconds;After treatment,the SIT of the treatment group was increased by an average of 5.92 mm,and is significantly higher than the control group increased by an average of 1.57mm;The SIT of right eye increased by an average of 5.46 mm,and is significantly higher than thecontrol group increased by an average of 1.74 mm.The difference between the two groups was statistically significant.(P<0.0001).It indicates that each index improvement of treatment group is significantly better than control group.It’s better to use Zengye Decoction combined with Sodium Hyaluronate eye drops to treat the patients than simply use Sodium Hyaluronate eye drops to treat patients in the improvement of each index.The effective rate of the treatment group is 100%,which is significantly higher than that of control group(57.1%)(P<0.0001).Conclusion: The treatment effect of Zengye Decoction combines with artificial tears is better than pure artificial tears.It can effectively improve tear secretion of lung yin deficiency syndrome of dry eye patients,and can improve symptoms and signs of dry eye patients among dermatology. |
