Objective:Bullous pemphigoid,which affects old patients,is a kind of autoimmune disease.Its histopathology shows subepidermal blisters,deposition of basement membrane antigen-antibody complex,and infiltrating inflammatory cells which are mainly neutrophils,EOSinophils andlymphocytes in the blisters and Dermal papilla.BP patients need topical systemic application of glucocorticoid to control disease for a long time.However,elderly patients are often associated with various underlying diseases such as cerebrovascular disease,heart disease and diabetes,and long-term use of corticosteroids may cause side-effect(such as osteoporosis,adrenal insufficiency etc.)which may aggravate the underlying disease,even increased mortality.Therefore,to develop a reasonable regimen is necessary for patients with BP.TO determine the severity of BP,clinician rely mainly on lesions area which lack of quantitative tests.At the same time,some BP patients show non-specific lesions such as urticaria or eczema like lesions at the onset of the disease.Therefore we recorded and analyzed some indexs in addition to the objective index of BP patients which are lesiong area index to evaluate disease severity,EOSinophilia leves in peripheral blood and serum titers of anti-BP180 and anti-BP230 Ig G autoantidodies.we also analyzed EOSinophilia leves in peripheral blood of BP patients with cerebralvascular disease,and serum titers of anti-BP180 and anti-BP230 Ig G autoantidodies,in order to provide the basis of the treatment plan for BP patients and BP patients with cerebralvascular diease.Method:A total of 101 untreated BP patients were enrolled in this study.The blood EOSlevels and the serum titer of anti-BP180 and anti-BP230 antibodies were recorded.The correlation of disease severity,the blood EOS levels and serum titers of anti-BP180 and anti-BP230 antibodies were analyzed retrospectively.Results:1.EOS increased in 68 untreated BP patients(67.33%).2.There was a positive correlation between the severity of the di-sease and the level of EOS in peripheral blood of 101 patients with BP before treatment.(P<0.001,r=0.436).3.There was a positive correlation between the severity of the di-sease and the serum levels of anti-BP180 antibodies in 34 BP patients d-etected antibody titers before treatment.(P<0.001,r=0.959).4.There was no correlation between the severity of the disease an-d the serum levels of anti-BP230 antibodies in 34 BP patients detected antibody titers before treatment.5.There was a positive correlation between the level of EOS in peripheral blood and the serum levels of anti-BP180 antibodies in 34 BP patients detected antibody titers before treatment.(P<0.05,r=0.55).6.There was no significant difference in the blood EOS level between BP patients with cerebrovascular disease and BP patients without cerebrovascular disease in 34 BP patients detected antibody titers before treatment.(P=0.56,t=-0.59).7.There was no significant difference in the serum levels of anti-BP180 antibodie s between BP patients with cerebrovascular disease and B-P patients without cere brovascular disease in 34 BP patients detected antibody titers before treatment.(P=0.062,t=-1.93).8.There was no significant difference in the serum levels of anti-BP230 antibodies between BP patients with cerebrovascular disease and B-P patients without cerebrovascular disease in 34 BP patients detected antibody titers before treatment.(P=0.1,t=-1.69).Conclusions 1.The level of EOS in peripheral blood of BP patients before treatment was increased,and it could be used as one of the indicators to determine the severity of the disease,and was positively correlated with the serum anti-BP180 antibody titers.2.The anti-BP180 antibodies can be used as an indicator for BP diagnosis and its titers in serum can be used as an one of the indicators to evaluate the severity of BP patients.3.The anti-BP230 antibodies can be used as an indicator for BP diagnosis but its titers in serum cannot be used as an indicator to evaluate the severity of BP patients.4.Cerebrovascular disease had no effect on blood EOS level,serum anti-BP180 antibody titer and serum anti-BP230 antibody titer in patients with BP.
