TM/TAT/PIC/t-PAIC Diagnosis Of Disseminated Intravascular Coagulation Prospective Clinical Study

Objective:1.To assess the sensitivity and specificity of TAT / TM / PIC / t-PAIC for DIC Diagnosis respectively2.To explore the sensitivity and specificity of TAT / TM / PIC / t-PAIC in the Diagnosis of DIC,3.To explore the value of TAT / TM / PIC / t-PAIC in DIC early diagnosis,Provide a new method for early diagnosis of DIC.Methods:Collected in 2014.10-2016.10 our hospital to meet the criteria for the assessment of DIC patients with 69 cases of all hospital data,All patients under the same conditions were collected on day 1,day 3,day 7,day 10 of venous blood,The four indexes of TAT,TM,PIC and t-PAIC were detected by high sensitivity chemiluminescence enzyme immunoassay,In order to assess the patient's disease situation evaluation,accurate diagnosis of DIC patients and grouping.acording to the 2012 DIC diagnosis and treatment of Chinese experts consensus diagnosis and diagnosis of DIC patients,among them DIC patients(case group)in 31 cases,NonDIC patients(negative control group)38 cases.our hospital exam department detection of routine coagulation index.to determine the reference standard,38 patients(18 years old to 70 years old)with physical examination in our hospital were used with high sensitivity chemiluminescence enzyme immunoassay for the Detection of TAT,TM,PIC,t-PAIC.SPSS19.0 was used to analyze the data,All data are expressed as mean ± standard deviation,Setting P <0.05 was significant.The area under the ROC curve> 0.5 is a valuable diagnosis.Results:1)TM diagnostic DIC sensitivity of 90.32%,specificity of 50%,There was a significant difference between the case group and the negative control group(P = 0.001),Case group/negative control group and healthy control group TM changes were significantly different(P = 0.000,P = 0.000);The area under the ROC curve is 0.702.2)TAT diagnostic DIC sensitivity of 80.65%,specificity of 50%,There was a significant difference between the case group and the negative control group(P = 0.043),Case group/negative control group and healthy control group TAT changes were significantly different(P = 0.000,P = 0.001);The area under the ROC curve is 0.653.3)PIC diagnostic DIC sensitivity of 83.87%,specificity of 26.32%,There was no significant difference in PIC between the case group and the negative control group(P = 0.252),Case group/negative control group and healthy control group PIC changes were significantly different(P = 0.003,P = 0.000);The area under the ROC curve is 0.551.4)T-PAIC diagnostic DIC sensitivity of 83.87%,specificity of 65.79%,There was a significant difference between the case group and the negative control group(P = 0.007),Case group/negative control group and healthy control group TM changes were significantly different(P = 0.000,P = 0.01);The area under the ROC curve is 0.748.5)TAT / TM / PIC / t-PAIC four indicators together with the diagnosis of DIC sensitivity of 61.29%,specificity of 86.84%,The area under the ROC curve is 0.741.Conclusion:Although the specificity of TAT / TM / PIC / t-PAIC diagnostic DIC is not high,but the sensitivity is high,the rate of missed diagnosis is low,It can be used as a diagnostic laboratory indicators of early DIC;The diagnosis of early DIC has a higher value,Four together with the diagnosis of DIC specificity is higher,Misdiagnosis rate is low,Sensitivity is not low,The diagnostic value for the DIC is also higher,The three indicators above(TAT/TM/t-PAIC)are of high value for early diagnosis of DIC,.to the DIC diagnosis,treatment and prognosis is important.
