OBJECTIVE:To evaluate the effectiveness and safety of the holmium laser enucleation of the prostate(HoLEP)and transurethral resection of the Prostate(TURP)in the treatment of benign prostatic hyperplasia(BPH).METHODS:We searched Pubmed?Elsevier ScienceDirect,Wanfang and CNKI for randomized controlled trials(RCT)comparing HoLEP with TURP.Comparable data were extracted from eligible studies and pooled for meta-analysis using RevMan5.3.Perioperative parameters recorded included operation time,blood transfusion,resected tissue weight,catheter time and hospital stay.Patients were followed postoperatively with maximum flow rate(Qmax),postvoid residual urine(PVR),international prostate symptom score(IPSS)and AUA symptom score(AUA-SS)and and the urethral stricture complication rate.RESULTS:Nine papers involving 1081 patients were finally obtained after screening,including article 4 in Chinese,article 5 in English.Meta-analysis showed that:although the HoLEP technique requires more time to perform than standard TURP,it is associated with significantly shorter catheterization time and hospital stay.Compares PVR reduction and demonstrates have statistically significant differences between HoLEP and TURP.Complications are similar with the 2 procedures.CONCLUTION:Holmium laser enucleation of the prostate(HoLEP)is a more safe and minimally invasive technique that can guarantee results at the Meta-analyses that are similar to those of standard transurethral resection in terms of relief of subjective symptoms and urodynamic findings due to bladder outlet obstruction in patients with BPH. |
