Background: Knee osteoarthritis(KOA)is a common chronic disease in the clinic,which is mainly manifested by joint degeneration.The disease is common in the middle and old age population,making the patient suffer from the great mental stress caused by repeated pain and the panic of the disabled.Experiments show that knee joint synovitis plays a key role in the development of osteoarthritis of the knee.Cartilage degradation causes inflammatory reaction of synovium,and inflammatory synovium further releases inflammatory cytokines,cytokines and proteases,which react on cartilage and aggravate cartilage destruction.If it can control synovitis in time and effectively and reduce the degradation of cartilage,it can effectively slow down the development of knee osteoarthritis.The external treatment of traditional Chinese medicine can be used to absorb the effective components of Chinese herbal medicine through the absorption of the skin and function in the affected area,which has the advantages of convenience,safety,economic application and so on.Xiaoyu jiegusan is a topical medicine national name of old Chinese Ding e founded by Professor,orthopedic treatment in the First Affiliated Hospital of Anhui University of traditional Chinese medicine widely used in knee osteoarthritis,can effectively relieve knee osteoarthritis early joint pain,swelling,clinical curative effect,but it is unclear its mechanism of action.Objective: Through the establishment of early knee osteoarthritis model in rats,observe the external application of Xiaoyu Jiegu San "on the matrix of knee osteoarthritis synovial tissue metalloproteinase 1,matrix metalloproteinase 3 expression,and observe the knee joint synovial tissue pathological changes before and after treatment,to explore the mechanism of removing blood stasis with bone powder for the treatment of knee osteoarthritis the.Methods: A total of 3 month old healthy male SD rats,weighing 75 250~300g,were randomly divided into 14 groups: normal group and sham operation group.A total of 75 healthy male SD rats,weighing 3 month old 250~300g,were randomly selected from 14 healthy male rats.They were randomly divided into normal group and sham operation group,and were divided into cage rearing group.The remaining 47 rats were operated on to establish the model of knee osteoarthritis.The sham operation group is only to verify whether the surgical trauma factors affect the results of the experiment.At the beginning of the week after the operation,every group of rats were run at regular intervals,and the right limbs were forced to move to the right lower limb for a period of30 min for 4 weeks.After 4 weeks,5 were killed randomly in the model rats,and the success of the model was detected.After the modeling,the operation model of 42 rats were randomly divided into 3 groups: model group,Xiaoyu jiegusan group(Chinese medicine group),diclofenac group(Western medicine group),14 rats in each group,each group of experimental animal drug dosage according to experimental design.After4 weeks of administration,all the animals were killed,the specimens were extracted and the HE staining pathological sections were made and observed under light microscope.Meanwhile,the relative expressions of MMP-1 and MMP-3 in synovial tissues were detected by fluorescence quantitative polymerase chain reaction(FQ-PCR),Western blotting(WB)and enzyme-linked immunosorbent assay(ELISA),respectively,and analyzed statistically.Results: After a course of treatment,the m RNA content of MMP-1 and MMP-3 in the synovial tissue of the model group was higher than that of the other groups(P<0.05).There was no significant difference between the MMP-1 Chinese medicine group and the western medicine group(P>0.05).The expression of MMP-3 group was slightly lower than that in western medicine group,the difference was statistically significant(P<0.05).Western blotting analysis showed that MMP-1 and MMP-3 were highly expressed in synovial tissues of the model group,and the difference was statistically significant compared with the traditional Chinese medicine group and Western medicine group(P<0.05).There was no significant difference between the Chinese medicine group and the western medicine group(P>0.05).Enzyme-linked immunosorbent assay showed that the two proteins of MMP-1 and MMP-3 were secreted into the synovial tissue of rats in the model group,and the difference was statistically significant compared with the traditional Chinese medicine group and Western medicine group(P<0.05).There was no significant difference between the MMP-1 Chinese medicine group and the western medicine group(P>0.05).The expression of MMP-3 group was slightly lower than that in western medicine group,the difference was statistically significant(P<0.05).Synovial tissue pathological observation showed that synovial membrane in model group was significantly proliferated,accompanied by a large number of inflammatory cells infiltration and significant proliferation of peripheral vessels.Compared with model group,synovial hyperplasia in Chinese medicine group was relatively small,a small number of inflammatory cells infiltration and a small amount of vascular proliferation.The synovial hyperplasia of the western medicine group was less than that in the model group,and the vascular proliferation and inflammatory cell infiltration were almost equal to that of the Chinese medicine group.Conclusion: The experimental study found that the pathological changes of "Xiaoyu Jiegu San can effectively reduce the early rat model of knee osteoarthritis synovitis,and can significantly reduce the two key MMP-1 and MMP-3 protease destruction and degradation of cartilage matrix in synovial tissue of m RNA expression,synthesis and secretion of protein The effect of traditional Chinese medicine and Western Medicine on the expression of MMP-1 in KOA synovial tissue is the same.In inhibiting the expression of MMP-3 in KOA synovial tissue,the Chinese medicine group is superior to the western medicine group.The mechanism may be that Xiao Yu Jie Gu San can effectively alleviate synovial inflammation,inhibit the expression of MMP-1 and MMP-3,reduce the degradation and destruction of articular cartilage,thereby alleviating the symptoms and disease progression of knee osteoarthritis.
