Objective: Knee Osteoarthritis(KOA) is a common clinical disease. According to yearsof clinical experience in treating KOA, Xiaoyu Jiegu powder, wihch were developed ashospital preparations by Professor Ding E, obtained well curative effect. This studymianly observe the clinical treatment effect of Xiaoyu Jiegu powder on early,metaphase KOA and its mechanism through clinical and experimental study.Methods:32mid-KOA model rabbits were randomly divided into four groups,determine the content of COMP and CTX-II in serum, observe the histomorphologychanges of knee articular cartilage before and after treatment, explore the possiblemechanism of Xiaoyu Jiegu powder.80cases of KOA patients were collectedsimultaneously, which were choosed from our department in2011-2013. All the caseswere randomly divided into two groups according to treatment method, statisticalanalysis, and observe the clinical effect of Xiaoyu Jiegu powder..Results:1Experiment reaserch(1) COMP: Compared with normal group, COMP content was significantly highercontained in modal group. Compared with modal group, COMP in treatment group andcontrol group decreased(P <0.01). The cotent has no statistical differences betweencontrol group and model group (P>0.05). Compared with control group, the treatmentgroup has obviously lower COMP content (P <0.01).(2) CTX-Ⅱ: Compared with normal group, serum CTX-Ⅱcontent in model group ishighly expressed. Compared with modal group, treatment group and control group haslower amount of CTX-Ⅱ, the differences are statistically significant (P <0.01). Compared with control group, the treatment group has statistically significant difference(P <0.01).(3) Mankin score under the microscopic observation: Compared with normal group,Mankin score was significantly higher in model group. Compared with modal group,Mankin score of treatment group and control group got an apparent decline but stillhigher than that of normal group, and the difference is significant (P <0.01). Comparedwith control group, the treatment group has statistically significant difference (P <0.01).2Clinical observation(1) After30days’ treatment, Xiaoyu Jiegu powder got clinical total effective rate of95%, that was better than the total effective rate of82.5%of Voltaren control group(P<0.05);(2) After30days treatment, compared with before, knee joint pain symptoms wassignificantly reduced, the differences are statistically significant (P <0.01). Thecomparison between the two groups have no statistical difference (P>0.05).(3) After30days’ treatment, joint swelling degree, mobility, and20meters walkingtime were significantly improved. Both groups have significant difference with beforetreatment (P <0.01), The treatment group has obviously efficacy than the control group(P <0.01).Conclusion:(1) COMP, CTX-Ⅱin serum play regulatory roles in KOA formation;(2) Xiaoyu Jiegu powder can effectively inhibit the expression of serum COMP, CTX-Ⅱ(P <0.01), and the effect of inhibition on CTX-Ⅱwas better than Voltaren (P <0.05).Voltaren had no effect on expression level of COMP in serum(P>0.05).(3) Xiaoyu Jiegu powder and Voltaren can both obviously improve the pathologicalchanges of knee articular cartilage in mid-KOA model rabbits (P <0.01), and the effecton bone by Xiaoyu Jiegu powder is obviously better than the effect of Voltaren (P<0.01). (4) Xiaoyu Jiegu powder have curative clinical effects on treat early-metaphase KOApatients, and the overall effect is better than Voltaren (P <0.05).(5) Xiaoyu Jiegu powder and Voltaren can both effectively alleviate the pain symptomsof KOA patients (P <0.01), have similar effect on analgesic effect (P>0.05). XiaoyuJiegu powder can significantly improve the patients’ walk ability, joint activity, andlower the degree of knee joint swelling, the effect is better than Voltaren (P <0.01)。...
