Diagnostic Value And Coincidence Analysis Of PSA And Related Markers And TRUS For Prostate Cancer

Objective: Analysis of prostate specific antigen(PSA)and related markers,transrectal ultrasonography(TRUS)biopsy positive rate and each markers in The coincidence of diagnosis in prostate cancer.Methods: Clinical data of 913 patients who underwent transrectal ultrasound guided prostate biopsy were analyzed retrospectively,Analyzed the diagnostic value of PSA and its related indicators,TRUS for prostate cancer,and compared the coincidence rate of PSA and TRUS in predicting prostate cancer.All the data were analyzed on the SPSS 21.0 for Windows statistical software package.Results: 913 patients who underwent transrectal ultrasound-guided prostate biopsy,365 of them were diagnosed as prostate cancer by biopsy,the overall positive rate was41.4%.There was significant difference in age,PSA,f/t PSA,PSAD and prostate volume between positive group and negative group.The age of the patient is between 43~94 years,the average was 70.82±8.51,the age of high incidence of prostate cancer was 60~80years old,accounting for 70.41% of the prostate cancer.The positive rate of PSA,TRUS,PSAD,f/t PSA in the diagnosis of prostate cancer was respectively 50.54%,50.62%,54.26%,46.78%;the Sensitivity was respectively 76.99%,78.36%,85.48%,73.70%;the Specificity was respectively 49.82%,49.09%,52.01%,44.16%;Diagnostic coincidence rate was 60.68%,60.79%,65.39%,55.97%.The positive rate,specificity,sensitivity and coincidence rate of PSA,PSAD,f/t PSA combined with TRUS were 64.45%,69.02%,59.15%,77.55%,77.55%,73.54%;61.10%,67.12%,57.53%;70.97%,73.38%,67.14%?When t PSA <4 ng/ml,4~10 ng/ml,10~20 ng/ml,>20 ng/ml,The positive rate of biopsy was respectively 17.57%,25.09%,28.81%,67.41%.For PSA 4~10 ng/ml and 10~20 ng/ml,when PSAD and f/t PSA were introduced,the positive rate of biopsy was 41.49%,32.12%and 35.59%,34.59% respectively.The main manifestations of prostate cancer are hypoechoic nodules under Ultrasonic image,The maximum diameter of nodules is more than 1cm.There was a negative correlation between prostate volume and positive rate of prostate biopsy.Conclusions:1.The majority of patients with prostate cancer are older than 50 years old,the age of high incidence of prostate cancer was 60~80 years old,therefore,for men over the age of50,screening for prostate cancer is significant.2.There is a negative correlation between prostate volume and prostate cancer detection rate,for larger volume prostate,increasing the number of sampling needles can improve the positive rate.3.The main manifestations of prostate cancer was hypoechoic nodules under Ultrasonic image,but most hypoechoic nodules were not prostate cancer.4.The detection rate of prostate cancer was affected by a variety of factors,it is similar that the detection rate and coincidence rate of PSA,PSAD and TRUS for prostate cancer.Combined with TRUS can effectively improve the diagnostic coincidence rate of prostate cancer.5.In terms of positive rate,PSA 4~10ng/ml and 10~20ng/ml was almost the same,therefore,the PSA grey area can be extended to 4~20ng/ml.The value of PSAD diagnosis is better than that of f/t PSA in PSA grey area.
