Objective: A retrospective study was conducted to observe the short-term clinical efficacy and safety of combination of Deep and Superficial modes of fractional CO 2 laser in the treatment of atrophic acne scar.Methods: From January 2017 to August 2017,twenty patients(8 females,12 males,Fitzpatrick skin type ? ~ ?)with atrophic scars of the treaded by fractional CO 2 Laser in the Seventh People's Hospital of Shenyang were reviewed retrospectively.All patients with age from 17 to 31 years old(average age 23.2 3.6 years).In this study there has 68 scars treated.The used of the treatment of atrophic scar patients combined by Superficial mode and Deep mode of laser system with wavelength of 10600nm:Firstly,the Superficial mode was selected for the shallow grinding of the sharp edges of the atrophic scar,with a coverage rate of 40% and 50 to 100mJ/cm2 for the energy density.Then according to the type of lesions selected treatment using energy density from 16.5 to 30.5mJ / cm,density coverage from 5% to 10% and 300 Hz for frequency which witched the Deep mode scan at the lesion again.The interval of treatment was 2 months and 3 times as one treatment course.The regions of patients which was treated acroding to the acne scars ECCA Grading scale score to get the scores and then analysising the decreasing trend of total scores of all acne scars before and after treatment,whether has any significant difference.The improvement of patients before and after treatment was accorded to the four-stage scale of acne,and the images of patients before and after treatment were obtained by using Antera 3D skin imaging system that evaluating by two fixed dermatologists.Objective analysis of volume changes of atrophic acne scar before and after treatment by using its image analysis system in the same site.After each treatment,the patients were asked about their satisfaction and downtime.Futhermore,the VAS scores,incidence and duration of adverse reactions were recorded.Then proceed with security analysis.Results: The experiment was included 20 patients with atrophic scars which a ll of them obtain the follow-up datas,a total of 68 atrophic scars were treated.T he morphology,diameter,depth and color of scars have been varying degrees of improvement after treatments.The preoperative score of ECCA Grading scale was(42.35 15.22),postoperative score decreased to(23.2 13.40)points,P <0.05,the difference was statistically significant.Among them 55% had an improve ment from 51 to 75%,five of patients improved by more than the percentage o f 75 and the average improvement was(63%19.25%).The average decrease o f cicatricial volume index of patients with local atrophic acne was(59.1% + 21.9%).The satisfaction rate of patients with statistics increased with the increase of the frequency of treatment,the last follow-up total satisfaction rate is 90% which very of satisfied was 62%,the rate of dissatisfaction was 0.The VAS sco re was(5.05 1.071)points,and all of the patients saidpaincan be tolerated.The postoperative edema and erythema was appeared of all patients,all edema was f aded within 3 days,all erythema was faded within 2 months,5 cases appeared e xudation in 3 days,10 cases of patients with transientpigmentation which was su bsided within 2 months.No severe postoperative adverse reactions occurred in th e patients.The average length of downtime was(5.65 1.423)days.None of the patients reprsented that the treatment affected the daily work,quality of life and l earning seriously.Conclusion:The patients with atrophic scar treated by Ultrapulse CO2 laser combined with Superficial mode and Deep mode had a marked improvement in the ance scars.There has a high satisfaction rate and no serious complications during follow-up.The short-term curative effect and safety of the super-pulsed CO2 laser were reliable.
