Objective:To investigate the feasibility and value of predicting diagnosis of dif ferentiated thyroid cancer by thyroid peroxidase antibody(TPOAb), thyroglobulin ant ibody(TgAb) and thyroid stimulating hormone(TSH). Methods:A retrospective surv ey was carry out in400patients with nodular thyroid disease who for the first ti me underwent thyroidectomy, from January2009to December2011in our hospital. DTC and benign group (n=200) were diagnosed by histopathological examination. Test two groups with levels of preoperative serum thyroglobulin antibody(TgAb), th yroid peroxidase antibody(TPOAb) and TSH. It is divided into negative group and positive group by the different range of TgAb, TPOAb; at the same time it is divi ded into low, normal, high three groups according to different range of TSH, and analyzed data. Results:Preoperative serum TgAb level was154(48)μU/ml in DTC group, was higher than35(14)μU/ml in benign group, differences are statistically si gnificant (Z=-2.35, P=0.019). According to the levels of serum TgAb, for DTC gr oups,34cases in positive (17.0%), for benign group,9cases in positive (4.5%), c omparing the positive rate of two groups, differences are statistically significant (χ2=18.56, P<0.01). Make a comparison between preoperative serum TPOAb leve1was51(17)μU/ml in DTC group and33(13)μU/ml in benign group, there was no statistically significant difference (Z=-0.92, P=0.36). TSH level was2.95(2.22)uU/ml in DTC groups, was higher than1.92(1.68)uU/ml in benign groups, differences are statistically significant (Z=-6.52, P<0.01). According to the levels of serum T SH, for DTC group and benign group, comparing the two groups, differences are s tatistically significant (Z=-3.67,P=0.0002). In the single factor analysis, TgAb and TSH have correlation with DTC. In the multivariate analysis, elevation of TgAb a nd TSH entered the regression model. Level of serum TSH was2.95(2.22)μU/ml i n DTC with neck lymph node metastasis was higher than2.66(1.98)μU/ml in DTC without neck lymph node metastasis, differences are statistically significant Z=-6.11, P<0.01). Conclusion:Preoperative serum TgAb and TSH are danger indicators for malignant thyroid nodules, and it need a large number of prospective studies c onfirms that whether they can be used as prediction index of differentiated thyroid cancer. Higher TSH level is associated with lateral lymph node metastasis.
