Data Mining And Clinical Study On The Treatment Of Intertrochanteric Fracture By Traditional Chinese Medicine

ObjectiveThe traditional Chinese medicine(TCM)inheritance support platform(V2.5)was used to analyze the composing principles of the prescriptions for the treatment of intertrochanteric fracture(IF).At the same time,to discuss the promotion of Qianggu capsule on blood biochemical indexes during the IF healing,the clinical observation of Qianggu Capsule was implemented in the early repair process of IF.The blood biochemical indexes included serum calcium(Ca),phosphorus(P)and alkaline phosphatase(ALP).MethodsThe data of prescriptions in treating IF from literature between January 2000 and December 2017 was collected and put into the TCM inheritance support platform,and then the data was analyzed based on revised mutual information,principle analysis,complex system entropy cluster and unsupervised hierarchical clustering composing principles.The patients with femoral intertrochanteric fracture surgery were collected in the First Affiliated Hospital of Guangzhou University of Chinese Medicine from May 2017 to January 2018.According to whether or not to take Qianggu capsule,they were divided into observation group and control group.The general information and hospital records of patients,including Ca,P,ALP,were measured in 24 hours of admission and 7 days after the operation.ResultsBased on the analysis of 211 prescriptions,the frequency,scope and meridian entry distributions of each Chinese herb were found,association rules among herbs included in the database were computed,9 new prescriptions were explored from the database.40 cases of effective patients were collected,of which the observation group and the control group were 20,and the basic data of the two groups were not statistically different.Comparison of serum total Ca,P concentration of two groups after operation,there was no significant difference between the groups(t = 1.22,P = 0.231;t = 1.64,P = 0.109).Ca,P product after operation,the observation group was higher than the control group,and the difference was statistically significant(t = 2.227,P = 0.032).Compared with the control group,the serum ALP concentration in the observation group increased after the operation,and the difference was statistically significant(t = 2.078,P = 0.044).The difference between the two groups before and after operation indicated that the differences were statistically significant(P <0.05).ConclusionThe methods of replenishing qi,warming yang and enriching blood,activating blood and removing blood stasis,and relieving pain were the key processes for the treatment of IF.The meridian entry distributions of herbs were mainly treated with the liver meridian.Most of the drug properties belong to Wen,Ping,Gan,Ku,Xin.Drynaria and Dipsacus,Safflower and Taoren,Safflower and Radix Paeoniae rubra were the high frequency used drugs combination.Qianggu capsule could maintain the concentration of Ca and P after operation of IF effectively,and the postoperative Ca,P product and serum ALP activity were increased relatively,which may conducive to the healing of fracture.
