Background:In recent years,the study of hypertension in modern medicine has mainly focused on the debate on its antihypertensive strategy.More and more evidences prove the benefits of intensified antihypertensive regimens.However,the incidence of adverse events caused by intensified antihypertensive treatment is increasing.Physical problems such as increased physical discomfort and poor medication compliance are also not to be ignored.Chinese medicine,which is characterized by its overall reconciliation,favors the idea of“moderately lowering blood pressure'' and has its advantages in the treatment of hypertension.The theory of liver wind in Chinese medicine has a long history.It plays an important role in helping us understand the pathogenesis of hypertension.Liver wind is the main line of pathological evolution of hypertension,and it has an internal consistency with the liver-yang hyperactivity.And liver wind,fire,deficiency,and tanshi usually work together.Based on this feature,my tutor often use the method of Qingjiang,Tiaobu and Hehua to treat hypertension.Tianma Gouteng Yin is a commonly used formula for the treatment of hypertension of liver-yang hyperactivity.For a long time,we have used the liver-yang hyperactivity as the main zheng of hypertension.However,based on the zhengxing studies carried out in the previous period,we found that the liver-yang hyperactivity zheng in hypertension patients is not the most important syndrome type.Hypertension patients often have evidence of tan,xueyu,etc.Some studies have found that the yangkang zheng has higher variability of blood pressure than other zheng,and the risk of stroke events is higher.Patients with liver yang hyperactivity often have other diseases such as hyperlipidemia,diabetes,etc.The intervention of these factors can contribute to the overall control of hypertension risk factors.In addition,patients with liver-yang hyperactivity zheng are mostly concentrated between the ages of 30 and 60.This age group is the main source of labor in the society,and their disease duration is usually between 5 and 15 years.This group of people needs timely and effective intervention.Raising the quality of life of the people at this stage will help ease the pressure on the aging stage and have a strong social reality.Therefore,we conducted a clinical hyperactivity zheng,so as to provide more means for the intervention of hypstudy of Tianma Gouteng Yin in treating hypertensive patients with liver yang ertension.Objective:To evaluate Tianma Gouteng Yin joint Norvasc treatment of essential hypertension of liver-yang hyperactivity in the promotion of standard blood pressure,improve symptoms,improve quality of life,improve outcomes,and benefits of blood lipids,inflammatory response.Methods:The 120 eligible patients enrolled were randomly assigned to the treatment group(Tianma Gouteng Yin granules 1/time of 2 times/day + Norvasc 5mg 1/day)and the control group(Norvasc 5mg 1/day),60 people in each group.In addition to interventional drugs,no other drugs are used in principle.Both groups perform the same hypertension education and management.The treatment process was 12 weeks and followed up for 1 month.After randomization,drugs were distributed every two weeks for 12 weeks.The results of the two groups of patients before and after the intervention were compared and analyzed,such as blood pressure,biochemical tests,TCM Syndrome Rating Scale,Dukes Hypertension Life Quality Scale,and so on.Results:A total of 113 patients were completed in this research.All of them were unable to adhere to the medication because of traveling abroad.In this study,there were no statistically significant differences between the two groups of patients in terms of age,gender,duration of illness,baseline blood pressure,and other comparable data.The blood pressure levels after treatment in both groups were lower than those before treatment,with statistical differences.Among the observation group,the effect was more obvious in the observation group than in the control group.There was a significant difference between the two groups,P<0.05,statistically significant.By counting the scores before and after treatment in the two groups,the efficacy index was calculated.The total effective rate was 94.64%in the observation group and 77.19%in the control group.Before treatment,the systolic blood pressure of the observation group was 160.74 ± 26.85mmHg,and the diastolic pressure was 99.52 ± 10.87mmHg.After treatment,the systolic blood pressure was 130.16 ±14.38mmHg and the diastolic blood pressure was 75.61 ± 7.92mmHg in the observation group;the systolic pressure was 158.81 ±21.55mmHg and the diastolic pressure was 97.85 ± 11.48mmHg in the control group before treatment;After treatment,the systolic blood pressure of the control group was 142.32 ±20.57mmHg,and the diastolic pressure was 85.54±8.86mmHg.The difference between the two groups in the efficacy of TCM syndromes was significant(P<0.05).In the quality of life scale,there was no significant difference between the two groups before treatment(p>0.05).After treatment,within the group comparison,the scores of the two groups of patients were significantly improved(p<0.05)before treatment.In the comparison between groups,compared with the control group,the difference in observation group on TC was statistically significant(p<0.05),while the remaining blood lipids were not statistically significant difference between the groups(p>0.05).The CRP levels after treatment in both groups were improved compared with before treatment,in which the C-reactive protein level in the observation group decreased significantly compared with that before treatment(p<0.05);the C-reactive protein level in the control group also decreased after treatment.The difference was not statistically significant(p>0.05);from the point of view between the observation group and the control group,the difference was statistically significant(p<0.05).After treatment,homocysteine levels decreased in both groups,but the HCY in the observation group decreased more significantly than before treatment(p<0.05).Between groups compared with the control group,the difference between the two groups was statistically significant(p<0.05).Changes in safety indicators before and after treatment within and between groups were not statistically significant.Conclusion:Tianma Gouteng Yin combined with Norvasc antihypertensive treatment is more effective than the use of Norvasc alone,and it is also superior to the western medicine group in relieving the symptoms of TCM.There is no significant difference in improving the quality of life between the two groups.The combination of the two drugs can help promote the blood pressure of patients with essential hypertension of liver-yang hyperactivity to reach the standard value of blood pressure,improve the syndromes of traditional Chinese medicine,and can regulate blood lipids and reduce the levels of C-reactive protein and homocysteine to some extent.,and no toxic side effects.The good antihypertensive effect of Tianma Gouteng Yin combined with Norvasc may also benefit from the improvement of these indicators.In conclusion,Tianma Gouteng Yin combined with Norvasc has obvious synergistic effects and is worthy of further promotion and use.
