Objective: To discuss the differentiation of clinical characteristics and diagnosis and treatment between benign and malignant solid pseudopapillary neoplasm of the pancreas.Through researching the relationship between the tumor malignant degree and clinicopathological characteristics such as the size of the tumor or capsular invasion,It can be better to define the diagnostic criteria of malignant SPNs and give an early intervention or appropriate surgical plan to improve the prognosis.Methods: A total of 39 during January,2012 to July,2017 pathologically diagnosed SPN were included.Analyzed the clinical characteristics,imaging features,treatment methods and prognosis between the benign and malignant SPN patients.Results: Among the 39 patients,there are 27 benign SPN and 12 malignant SPN,the mean age of(35.3 + 12.1)years,the main clinical manifestations were asymptomatic physical examination findings or upper abdominal pain.CT,MRI is of great help for disease diagnosis.Tumor capsule whether complete,proportion of cystic or solid component in tumor,and tumor size in SPN was statistically difference between benign and malignant,but the gender,blood type,the position of the tumor,tumor morphology,and whether tumor with calcification,bleeding or pancreatic duct expansion have no statistical difference.Conclusion: capsule incomplete and the largest diameter > 5cm may suggest malignancy of SPN,and near-solid tumors may be indicative of benignancy.Surgery is the crucial and most effective treatment for SPN,and we suggest that routine radical operation for malignant SPN.
