| ObjectiveTo observe the clinical effect of Zusanli point injection combined with conventional western medicine on acute exacerbation of chronic obstructive pulmonary disease(AECOPD),Comprehensive evaluation of TCM syndrome score,immune index,inflammatory index,blood gas index and so on,and to explore the application value of Zusanli Point Injection in the treatment of AECOPD,so as to provide the clinical basis for the use of AECOPD.MethodsIn this study,70 AECOPD patients who were hospitalized in Department of respiration of the First Affiliated Hospital of Guangzhou University of Chinese Medicine from May 2017 to February 2018 were randomly divided into35 cases in the control group and the treatment group by random digital table method.The control group was treated with a routine treatment scheme,the treatment group was treated with acupoint injection therapy on the basis of the control group,and the drug selected Chuan Ke injection,and the acupoints were selected on the bilateral Zusanli.The course of treatment was 10 days.The changes of TCM syndrome score,immune index,inflammation index and blood gas index were observed in the two groups before and after treatment,and the occurrence of adverse reactions during the treatment was recorded.Results1.Baseline situation:There was no statistical difference between the two groups before treatment(P> 0.05)in age,sex,course of disease,days of aggravation,TCM syndrome score,immune index,inflammatory index,blood gas index and other groups(P> 0.05).2.Clinical efficacy:The total effective rate of the treatment group was94.1%,while that of the control group was 78.8%.The curative effect of the treatment group was better than that of the control group,and the difference was statistically significant(P< 0.05).3.TCM syndrome score:After treatment,the TCM syndrome scores of cough,phlegm,wheezing and chest tightness were significantly lower than before treatment(P < 0.01).The score of TCM syndrome in treatment group after treatment was lower than that of control group,and the difference was statistically significant(P< 0.05).The score of TCM syndrome of coughing and phlegm in the treatment group was lower than that of the control group(P< 0.05).Group B,but the difference was not statistically significant(P>0.05).4.Immunological index:(1)Cellular immunity:After treatment,the CD3+,CD3+CD4+,CD3+CD8+ and Th/Ts were significantly higher in the treatment group than before the treatment(P < 0.05),and compared with the control group,the difference was statistically significant(P< 0.05).After treatment,CD3+,CD3+CD4+,CD3+CD8+ and Th/Ts in the control group were not significantly improved compared with those before treatment(P> 0.05).(2)Humoral immunity:After treatment,the difference of IgG,IgA,IgM,IgE,complement C3 and complement C4 was not statistically significant(P> 0.05).The index of complement C4 in the treatment group was significantly higher than that before treatment,and the IgE index was significantly lower than before the treatment,and the difference was statistically significant(P< 0.05).There was no significant difference in body C3 between before and after treatment(P> 0.05).5.Inflammatory markers:After treatment,WBC,NEU% and CRP were significantly improved in the two groups(P< 0.01).The difference of WBC,NEU% and CRP in the treatment group was not statistically significant(P>0.05).6.Blood gas index:After treatment,PaCO2 and PaO2 were significantly improved in the two groups(P< 0.01).The difference of PaCO2 and PaO2 in the treatment group was statistically significant(P< 0.05).7.Safety indicators:There was no significant difference in safety index between the two groups before and after treatment(P> 0.05).ConclusionZusanli point injection Chuan Ke injection can improve the immune function of AECOPD patients,improve the clinical symptoms and blood gas index of patients.It has certain positive and beneficial effects on the treatment ofAECOPD.At the same time,it has high safety and less adverse reactions,have certain clinical application value. |
