The Study Of Xiao-zhong-ding-tong Oral Mixture In The Prevention Of Postoperative Lower-extremity Deep Vein Thrombosis Of The Elderly With Hip Fracture

ObjectiveTo study the clinical efficacy of Xiao-zhong-ding-tong oral mixture in the Prevention of Postoperative lower-extremity Deep vein Thrombosis of the elderly with hip fracture,provide a reference for the preventions of postoperative lower-extremity deep vein thrombosis of the elderly with hip fractures with combined Chinese and western medicine.MethodsAll the subjects were treated from December 2015 to December 2017 in the Second Department of Osteology of Guangzhou Integrated Chinese and Western Medicine Hospital,Guangzhou University of traditional Chinese Medicine,for the elderly hip fracture(femoral neck fracture,interochanteric fracture or proximal),who were prepared to be treated by surgery.According to the random number table,the patients who meet the inclusion criteria randomly divided into the test group and control group.On the basis of fracture operation and perioperative routine treatment,the control group was only treated with Nadroparin calcium in postoperative anticoagulant therapy.The test group was treated with Xiao-zhong-ding-Tong oral mixture combined with Nadroparin calcium in postoperative anticoagulant therapy.The changes of Hemoglobin,fibrinogen and d-dimer were recorded on pre-operation,the first day,the third day and the fifth day after operation in both groups.The postoperative positive rate of DVT and the quantitative score of DVT symptom in both groups were measured on pre-operation and the fifth day after operation.ResultsThere were 292 cases in accordance with the inclusion criteria,145 cases in the test group and 147 cases in the control group.There was no significantdifference in the baseline data between the both groups after comparison.Compared with the preoperative DVT symptom quantification score,Ddimer,fibrinogen and hemoglobin of the cases.There was no significant difference between the two groups.There were 8 cases occurring DVT in the control group and 6 cases in the test group,the positive rate was 5.44% and4.13% respectively,but there was no significant difference between the both groups.Comparing the both group about DVT symptom quantificative score on postoperative fiffth day,the difference was statistically significant(P<0.05).There were significant statistically differences in Ddimer and fibrinogen in both group on postoperative first,third and fifith day(P<0.05).Making a comparison of both group about Hemoglobin on postoperative first,third and fifth day,the differences on postoperative third and fifth day were statistically significant(P<0.05),but not on postoperative first day(P>0.05).ConclusionXiao-zhong-ding-Tong oral mixture combined with Nadroparin calcium can effectively prevent the postoperative lower extremity DVT of the elderly patients with hip fracture,and its curative effect is similar to using LMWH alone.Furthermore,combined therapy has more advantages in improving the the DVT symptoms,changing blood hypercoagulability,reducing the sybthesis of Fib and the risk of postoperative bleeding.
