Clinical Intervention Of ‘Xuanji Acupuncture' On The Treatment Of Hematologic Toxicity After Platinum-based Chemotherapy In Non-small Cell Lung Cancer

ObjectiveBy prospective front-to-back control design,taking the chemotherapy toxicity as the entry point,this study aims to observe the effects of ‘Xuanji acupuncture'reduction on hematologic toxicity after platinum-containing chemotherapy in non-small cell lung cancer,and to investigate the effects of ‘Xuanji acupuncture' on gastrointestinal reactions,T-lymphocyte subsets,PS scores and quality of life in patients.MethodsAccording to the inclusion criteria,40 patients with non-small cell lung cancer who received platinum-containing chemotherapy were selected in study.All patients were from Fangcun Branch of Guangdong Provincial Hospital of Traditional Chinese Medicine.The experiment is divided into two phases and a washoutperiod phase between the two phases.The first phase was the combination of chemotherapy plus western medicine and the combination of the‘Xuanji acupuncture',‘Xuanji acupuncture' was given on the day before chemotherapy,once a day,retaining needle 30 min,5 days for one period of treatment;after the interval 21-28 days,the second phase was started,the second phase was a routine treatment for chemotherapy plus western medicine,as a blank control group.According to WHO and CTCAE criteria for the classification of adverse drug reactions,the international quality of life score,PS score and T lymphocyte subsets efficacy evaluation indicators.We compared the degree of hematotoxicity on the 3rd,5th,7th and 14 th days of each phase,and compared the changes of PS score,T lymphocyte subsets and quality of life before and after each stage of chemotherapy.We also need compare the degree of gastrointestinal response during chemotherapy.ResultsFirst,40 patients had different degrees of hematologic toxicity after two-phase treatment.However,after chemotherapy,two phase comparison,the difference of myelosuppression degree of WBC,NEUT,HB,PLT and so on were statistically insignificant,P >0.05.Second,40 patients had different degrees of gastrointestinal response after two-phase treatment.In the nausea response,two phases compared,the difference was statistically significant on the first day of chemotherapy,P <0.05.In the vomiting response,the difference were statistically significant both on the first day of chemotherapy and the second day of chemotherapy,P <0.05.In the loss of appetite,two phases compared,the difference between the two programs was statistically significant both on the first day of chemotherapy and on the second day of chemotherapy,P <0.05.There were no significant differences in other items.Third,before the chemotherapy of two phases,There were no significant differences in T lymphocyte subsets level(P >0.05).In terms of CD3+(%),CD3+CD4+(%),CD3+CD8+(%)and CD3+CD4+/CD3+CD8+ level,statistical analysis was carried on to compare the results before and after the chemotherapy in first phase,which suggest the difference is no statistically significant(P >0.05).But statistical analysis was carried on to compare the results before and after the chemotherapy in second phase,There were significant differences(P <0.05).Two phases compared after chemotherapy,the difference were statistically significant in terms of CD3 +(%),CD3 +CD4 +(%)and CD3+CD4+/ CD3+CD8+(P <0.05),but the difference were no statistically significant in terms of CD3+CD8+(%)(P >0.05).Fourth,before the chemotherapy of two phases,there were no significant differences in PS score.Statistical analysis was carried on to compare the PS score before and after the chemotherapy in first phase,which suggest the difference is no statistically significant(P >0.05).But statistical analysis was carried on to compare the PS score before and after the chemotherapy in second phase,there was significant differences(P <0.05).Two phases compared after chemotherapy,the difference were statistically significant in PS score(P <0.05)?Fifth,before the chemotherapy of two phase,there were no significant differences in quality of life scores(P >0.05).In terms of constipation,statistical analysis was carried on to compare the results before and after the chemotherapy in first phase,which suggest the difference is statistically significant(P < 0.05),while there were no significant difference in other quality of life scores(such as body,character,emotion,cognition,social,tiredness,shortness of breath,insomnia,economy,diarrhea).In terms of body,character,emotion,cognition,society,fatigue,insomnia and constipation,statistical analysis was carried on to compare the results before and after the chemotherapy in second phase,which suggest the difference is statistically significant(P < 0.05),while there were no significant difference in other quality of life scores(such as economy,shortness of breath,diarrhea).Two phases compared after chemotherapy,the difference were statistically significant in quality of life scores(such as body,character,emotion,cognition,society,tiredness,insomnia,economy),while there were no significant difference in other quality of life scores(such as shortness of breath,constipation,diarrhea).ConclusionThe‘Xuanji acupuncture' has no obvious improvement in the hematologic toxicity response after chemotherapy with platinum-containing chemotherapy for non-small cell lung cancer,but it can alleviate the digestive tract reaction,and can also reduce the damage of chemotherapy on T-lymphocyte subsets.At the same time,it improves the physical condition and improve the quality of life.
