| ObjectiveObserve the distribution of TCM constitution in patients with Nonalcoholic Fatty Liver Disease,to explore the correlation between TCM constitution classification and nutritional status and metabolic factors in patients with nonalcoholic fatty liver disease,it is hoped that it can provide a theoretical basis for the prevention and early intervention of non-alcoholic fatty liver disease,and provides a new idea for treating this disease from the perspective of TCM constitutional science.MethodThis study uses cross-sectional survey methods,and it was conducted on patients diagnosed with non-alcoholic fatty liver disease in outpatient and inpatient departments of Guangdong Provincial Hospital from September 2016 to February 2018.The TCM Constitution Survey was conducted using TCM Constitution Classification and Assessment Scale,and BMI,waist-to-hip ratio,body fat content,protein content,blood lipids,fasting blood glucose,and uric acid were collected from these patients,some of these patients were tested for metabolic factors(adiponectin).To explore the correlation between TCM constitution and nutritional status indicators and metabolic factors.Result(1)General situation: A total of 250 NAFLD patients were included in the study,of which 189(75.6%)were male and 61(24.4%)were female.The average age of the patients was 40.44±12.37 years and the average body mass index was 26.85±3.95 kg/m2,among them,36.4%(91/250)patients had elevated ALT,18%(45/250)patients had elevated fasting blood glucose,54.8%(137/250)hadhypertriglyceridemia,and 46%(115/ 250)With hyperuricemia,6.4%(16/250)had hypertension.(2)NAFLD’s TCM Constitution Characteristics: The 250 non-alcoholic fatty liver patients included in this study were all biased constitutions,with the majority of them being damp heat,phlegm dampness,blood stasis,and qi deficiency,accounting for 90% of the total.There was no significant difference in the distribution of gender among patients of different TCM constitutions(P>0.05),the majority of patients with damp heat patients aged20-30 years accounted for 30.5%(26/85),and patients with phlegm dampness and blood stasis were mostly between 40-50 years old,accounting for 31.0%(22/71)and 40.0%(16/40)respectively,patients with qi deficiency were mostly 50-60 years old,accounting for 31.0%(9/29).(3)Correlativity of TCM Constitution and Nutritional Status Index in Patients with NAFLD: In terms of BMI,the levels of phlegm dampness and damp heat patients were equivalent to 27.93±3.78 kg/m2 and 27.16±4.50 kg/m2,respectively,This value was significantly higher than patients with Qi deficiency(25.32±3.32 kg/m2)and blood stasis(26.40±3.23 kg/m2)(P<0.05).In terms of actual weight and standard body weight ratio and waist-to-hip ratio,patients with phlegm and dampness were significantly higher than those with other TCM physiques.In terms of body fat content,the average level is 23.06±16.7%,The level of body fat in damp-heat patients was significantly higher than that of other TCM constitutions,and there was no obvious correlation between phlegm dampness and body fat.In terms of body protein content,the average level was 11.12±1.84%,and there was no statistically significant difference in the protein content levels among various Chinese medicine physical types(P>0.05).(4)Correlation of TCM Constitution and Metabolism Index in NAFLD Patients:There was Positive correlation between damp-heat and TG、UA levels.The TG level in patients with damp-heat constitution was significantly higher than that in patients with phlegm dampness and qi deficiency,and their serum UA levels were significantly higher than those in other constitution types.Qi deficiency constitution is negatively correlated with TG level,and its TG level is lower than other TCM constitution types.In terms of fasting blood glucose,there was no significant difference between patients with NAFLD of different TCM constitution types(P=0.489).Adiponectin was detected in 31 patients with NAFLD and Its average level is 2.82±1.87μg/L,and serum adiponectin levels in patients with phlegm dampness were significantly higher than those in patients without phlegm.ConclusionThe type of TCM constitution in patients with NAFLD is more common in terms of phlegm dampness,damp heat,blood stasis,and qi deficiency.There is a certain correlation between TCM classification and nutritional status indicators and metabolic indicators.Applying this correlation rule can provide better disease management for NAFLD patients. |
