Analysis Of The Clinicopathologic Features Of Primary Bilateral Breast Cancer

Objective:To investigate the clinicopathological characteristics of primary bilateral breast cancer.Methods:The clinicopathological characteristics and treatments of the 81 patients who received surgical treatments and pathological tests between 2004 and 2017 in our department with primary bilateral breast cancer were retrospectively reviewed and analyzed in this study.The data of the patients comes from our medical records management library.Results:The onset age of metachronous bilateral breast cancer?MBBC?is younger than that of synchronous bilateral breast cancer?SBBC??P=0.013?.The time interval between bilateral onset of MBBC is 8-151 months,with an average interval time of 43.5 + 35.8 months.At the time of initial diagnosis,SBBC is mostly found in postmenopausal women,yet MBBC is mostly found in premenopausal women?P=0.022?.At the same time,SBBC has more histories of pregnancy?P=0.001?.The average pregnancy of SBBC are 2₇4 times compared with that of MBBC are 1.81 times.The consistency of ER and PR in the SBBC group is higher than that in the MBBC group?P<0.05?.The negative rate and negative consistency of ER and PR in MBBC group are higher than those of SBBC?P<0.05?.Besides,the TNBCs are more common in the MBBC group than the SBBC group?P<0.001?.The first tumors of SBBC and MBBC are mainly in stage ?,while the second tumors of MBBC are mainly stage ?,which has statistically significant difference?P=0.040?.The size of neoplasms both decreased remarkably compared to the initial size after neoadjuvant chemotherapy applied in the SBBC and MBBC?P<0.001?.There is a significant difference in lymph node metastasis between the first and second cancers of the SBBC?P = 0.012?.The rate of adjuvant endocrine therapy for SBBC is higher?P<0.001?.However,as for BMI,production history,age of menarche,family history,history of basic diseases,basic characteristics of tumors,histological type,HER-2 receptor status,and other treatments except endocrine therapy,there is no significant difference between the MBBC group and the SBBC group.?P>0.05?Conclusion:The expression of ER and PR:in MBBC is lower,while the positive rate of hormone receptors?ER,PR?and consistency are higher in SBBC.More attention should be paid to the contralateral breasts in the young premenopausal women who already have unilateral breast cancers and a more advanced follow-up system should be established.
