Objective:In this study,miRNA-146 a and miRNA-185 were detected in 40 cases with malignant pleural effusion(MPE)and 41 cases with benign pleural effusion(BPE).The relative expression levels of miRNA-146 a and miRNA-185 were detected and our aim was evaluated clinical differential diagnosis of MPE and BPE.Methods:Real-time fluorescence quantitative PCR was used to detect the relative expression levels of miRNA-146 a and miRNA-185 in 40 patients with MPE and 41 patients with BPE.The Mann-Whitney U rankings were used to compare the groups of MPE and BPE,and compare the difference between the groups whether has statistically significant.By constructing the receiver operating curve(ROC curve)of the miRNA-146 a and miRNA-185,the sensitivity,specificity,and accuracy of the miRNA-146 a and miRNA-185 were calculated.And compared with the clinically mature cancer markers Carcinoembryonic antigen(CEA)and cytokeratin 19 fragment(Cyfra21-1).The miRNA-146 a and mi RNA-185 were used to evaluate the efficacy of MPE based on the area under the subject’s working curve(AUC).Result:1.Real-time fluorescence quantitative PCR showed that miRNA-146 a and miRNA-185 were significantly differently expressed in the MPE and BPE(P<0.01).Experimental data showed the expression levels of pleural effusion miRNA-146 a and miRNA-185 in patients with lung cancer were significantly lower than those with BPE patients.2.The ROC curve showed that the area under the curve(AUC)of miRNA-146 a was 0.816(95% CI,0.713-0.919),the sensitivity was 0.850,and the specificity was 0.805.The AUC of mi RNA-185 was 0.826(95% CI,0.729-0.924),with a sensitivity of 0.800 and a specificity of 0.854.The AUC of miRNA-146 a and miRNA-185 were lower than that of CEA 0.928(95% CI,0.861-0.966)but higher than that of CYFRA 21-1 0.734(95% CI,0.615-0.854).The sensitivity of the miRNA-146 a was higher than that of CEA and CYFRA 21-1,but the specificity was lower than that of CEA and CYFRA 21-1.The sensitivity of the miRNA-185 was higher than that of CYFRA 21-1 but lower than CEA,the specificity was lower than both CEA and CYFRA 21-1.The AUC with miRNA-146 a in combination with miRNA-185 was 0.814(95% CI,0.713-0.915),which was lower than the single diagnosis,the sensitivity was 0.763,which was lower than the single diagnosis,the specificity was 0.842,which was higher than miRNA-146 a but lower than miRNA-185.The AUC with miRNA-146 a in combined with CEA was 0.925(95% CI,0.857-0.992)and miRNA-185 in combined with CEA was 0.924(95% CI,0.856-0.992),which could improve the diagnostic value of malignant pleural effusion.The AUC with miRNA-146a、miRNA-185 in combined with CEA was 0.922(95% CI,0.844-1.000),there was no significant increase in the efficacy of any combination of miRNAs with CEA.3.The experimental results show that the expression of mi RNA-146 a and miRNA-185 in the pleural effusion of patients with lung cancer have no significant difference(P> 0.05)in the clinical parameters of age,sex and smoking history of patients Conclusion:The expressions of miRNA-146 a and miRNA-185 were detected expression in pleural effusions.The expression of miRNA-146 a and miRNA-185 in malignant pleural effusions was significantly lower than that in benign pleural effusions(P < 0.01).miRNA-146 a and miRNA-185 may become non-invasive markers in the differential diagnosis of malignant pleural effusion.
