Objective:Through epidemiological investigation on the case data of herpes zoster inpatients in a dermatological hospital of a city,this study analyzed the clinical characteristics of hospitalized patients and the related influencing factors of sequelae neuralgia,and put forward pertinent suggestions for the prevention and treatment of the disease in order to reduce the occurrence of sequelae neuralgia.Methods:A retrospective study method was used.Data of hospitalized patients with herpes zoster from January 1,2010 to December 31,2017 in a city dermatological hospital were collected.After eliminating the cases with extreme values and missing main indicators,486 cases were included.Their demographic characteristics,clinical data and related factors of sequelae neuralgia were analyzed.Access software is used to establish the database and export it to Excel 2007 for data collation.SPSS 20.0 is used for data analysis and processing.Normal distribution data is described by x±s,skewed distribution data is described by median and quartile spacing M(Q),and counting data is described by percentage.The data were analyzed by ?Z test,?2 trend test and rank sum test,and the test level was a=0.05.Results:1.Investigation results of herpes zoster inpatients1.1 Demographic characteristics:the proportion of herpes zoster inpatients was 486 in 2010?2017 years.There were 254 male patients(52.26%)and 232 female patients(47,74%).Most of the patients were over 60 years old.With the increase of age,the proportion of hospitalized people shows an upward trend.In terms of occupational distribution,the majority of hospitalized patients are retirees,followed by manual workers.1.2 Clinical manifestations:From 2013 afterwards,the number of inpatients increased year by year.There was no significant difference in hospitalization days between male and female patients(Z=0.187,P>0.05).The number of patients admitted to hospital in autumn was higher than that in other seasons.The incidence of thoracodorsal nerve area was the highest,followed by the trigeminal nerve area of head and face,and the lowest proportion of lower limb nerve area.The main inducing factors were biological factors and mental factors,accounting for 35.97%and 20.86%respectively.There were 207 herpes zoster patients with basic diseases,accounting for 42.59%of all cases.In terms of incidence types,gangrene type(41.77%),bullous type(39.71%)and herpes vulgaris type(18.52%)were in the order of composition ratio.There was no significant difference in the degree of acute pain between different age groups(H=3.229,P>0.05).2.Investigation results of telephone call back patients with herpes zoster sequela of neuralgiaThe occurrence of PHN in 306 patients with herpes zoster sequelae was analyzed,the incidence of PHN was 28.76%.There was no obvious relationship between gender and PHN(?2=0.879,P>0.05).The incidence of postherpetic neuralgia increases with the increase of patients'age,the deterioration of onset types and the aggravation of pain in acute stage.(P<0.05).Patients with prodromal pain have higher PHN than those without prodromal pain(?2=8.637,P<0.05).The incidence of PHN in the trigeminal region of the head and face was higher than that in the thoraco dorsal nerve area(?2=11.147,P<0.05).There was no significant difference in the incidence of postherpetic neuralgia among patients with different delayed visits and different underlying diseases.(P>0.05).Conclusion:1.Inpatients mainly concentrated in the elderly,mostly gangrenous herpes.2.In recent years,the number of hospitalized patients with herpes zoster has increased year by year,with obvious seasonality.3.The main sites of onset in hospitalized patients are the thoracic and costal nerve and trigeminal nerve,which can cause herpes by various inducements.4.There were significant differences in the incidence and types of underlying diseases among hospitalized patients of different age groups,while there was no significant difference in the degree of pain in the acute stage of herpes5.More than a quarter of the patients had PHN after discharge.The incidence of PHN was mainly related to age,prodromal pain,location,type and degree of acute pain.
