Objective:The anatomical study of the superior sagittal sinus,transverse sinus and the adjacent bridge vein was used to understand the significance of its anatomical structure in the treatment of tumor,trauma,and acute thrombosis of the important drainage veins or sinus,and to explore the replacement of the vein or vein in the treatment of these diseases.The feasibility of the sinus provides anatomical basis for cerebral venous bypass.Methods:1.The imaging data of 100 cases of head and neck angiography were selected in the hospitalized patients,and the origin,stroke,branch and variation of the superior sagittal sinus and transverse sinus were observed and recorded;2.The medical imaging data of 20 cases of MRV in the head and neck were selected in the hospitalized patients to observe the general behavior of the superior sagittal sinus,the number of the bridge veins in the superior sagittal sinus,the distribution of the bridge veins,and the observation and record of the advantages and disadvantages of the superior sagittal sinus which inflow into the bilateral transverse sinuses;3.10 adult cadaver head specimens with 10% Formaldehyde Solution fixed in the neck were selected to observe and record the general position of the upper sagittal sinus and transverse sinus in the cadaver head specimens,the number and direction of the vein,which provided the anatomical basis for the venous bypass.Results:1.Through the analysis of the imaging data of cerebral angiography in 100 hospitalized patients,the right dominant sinus was 48%,of which the right absolute dominance was 22%,a slight dominance was 26%,the left dominant sinus accounted for 17%,of which the left absolute dominance was 7%,the superior was 10%,the bilateral transverse sinus was 35%,and the general rule was right side dominance > bilateral balance > left advantage.This result is also confirmed in 20 cases of MRV imaging data.2.The length of superior sagittal sinus in 10 cadaver head specimens was 24.8 ±1.6cm.The length of upper sagittal sinus was 26.1±1.2cm in 10 random images of cerebral angiography,and it was found that the length of superior sagittal sinus was shorter than that in cerebral angiography.This may be associated with the upper sagittal sinus collapse in the cadaver head specimens.The elastic contraction of the blood vessel wall results in the shorter sagittal sinus length.The maximum horizontal diameter of the superior sagittal sinus in the 10 cadaver head specimens is 5.14±0.94 mm,the maximum inner diameter of the vertical position is 4.94±0.90 mm.The same measurement method is that the maximum horizontal diameter of the upper sagittal sinus is 5.95±0.86 mm,the maximum inner diameter of the vertical position is 5.54±1.16 mm and the upper sagittal sinus is in the human.The horizontal and vertical diameter of the position of the slots should be larger than the position of the coronary seams.With the upward and backward continuation of the superior sagittal sinus,the venous blood flow in the sinuses gradually increased,and the inner diameter of the sinus cavity was also consistent with the increase of the inner diameter of the sinus.The internal diameter of the left transverse sinus was 7.27±0.86 mm,and the inner diameter of the right transverse sinus was 8.37±0.80 mm.The inner diameter of the superior sagittal sinus was compared with the increase of the blood flow in the sinus.The inner diameter of the transverse sinus was obviously larger than the upper sagittal sinus.Comparison of the bilateral transverse sinus diameter can be drawn to people right transverse drainage was dominant.Conclusion:1.Morphological studies on the distribution of the superior sagittal sinus and transverse sinus,adjacent relationship,diameter and the distribution of the inbound veins provide anatomical basis for the clinical treatment of related diseases;2.When the superior sagittal sinus drained into the transverse sinuses at the sinuses,there was a rule of unilateral drainage venous sinus dominance,that is,the right side advantage,the bilateral balance power > the left side advantage;3.In the superior sagittal sinus,the fibrous cord similar to the "venous valve" structure is found in the sinus.The main types are valvular,trabecular and lamellar.The 3 kinds of fibrous cords have the largest value of valvular fibrous cord,which regulate the flow of the sinus into the sinus and prevent the blood flow into the bridge vein;4.For the treatment of neoplastic lesions involving the venous sinus,venous bypass provides the possibility of full resection of the tumor,and reduces the incidence of venous cerebral edema and cerebral infarction due to venous insufficiency after the operation;5.Cerebral vascular bypass technique reduces the difficulty in the treatment of venous related diseases,simplifying the complex operation and enriches the surgical approach to the treatment of these diseases.It is a safe and effective treatment.
