Objective: 1、Observation on the parasagittal backflow veins in digital subtraction angiography(DSA) the number morphology and distribution, 2、To discuss the importance and protection method of draining veins within or around parasinus and parafalx meningiomas in order to guide the microneurosurgery.Methods: 1、Retrospective analysis with 100 normal adults’ digital substaction angiography(DSA) of the Hospital. 2、Observed and measured the distribution and morphology of the Superior sagittal sinus and related backflow veins especially for the backflow veins of the motor areas of the brain. 3、Retrospective analysis was performed in 55 cases with parasagittal meningiomas received treatment in our department in the past five years regarding the demographic data, postoperative complications and the prognosis.Results: 1、The length of each segment of the superior sagittal sinus venous in DSA. Anterior segment of draining veins: 44.7 ± 18.9(24.9-79.3) mm, Anterior segment of draining veins void: 46.8 ± 18.7(24.8-106.8) mm, Posterior segment of draining veins : 75.3 ± 18.1(41.8-109.6) mm, Posterior segment of draining veins void: 71.1 ± 17.4(36.2-107.8) mm.Total length: 238.4 ± 26.7(178.2-294.4) mm. 2、The number of draining veins in anterior segment:2.65±1.03(0-4).The number of draining veins in posterior segment:5.98±1.40(3-9)3、The number of draining veins on the anterior segment of SSS on the left side:2.70±1.03(1-4).The number of draining veins on the anterior segment of SSS on theright side: 2.60±1.05(0-4). 4、The number of draining veins on the posterior segment of SSS on the left side:6.25±1.33(0-9).The number of draining veins on the posterior segment of SSS on the right side:5.70±1.45(0-9). 5、All patients were treated with microsurgery, and 54(98.2%) underwent total resection. Extent of surgery by Simpson criteria, 33(60.0%) were resected by grade I, 21(38.2%) by grade II and 1(1.8%) by grade IV. Permanent hemiplesia occurred in 3 due to surgical procedure. Three cases were complicated with evident postoperative brain swelling, and one was with intracranial hematoma. By Karnofsky performance score(KPS) after operation, 44 were between 100 and 90, 5 in 80 and 6, from 70 to 40.Conclusion: Successful surgery for removal of parasatiggal menigiomas is involved in study the anatomical features of Superior sagittal sinus and protection of the venous drainage.
