Objective: Iodine deficiency disorder(IDD)is a general term for a group of related diseases in the natural environment due to iodine deficiency,which results from embryo development to adulthood.The clinical manifestation is from light to heavy.The greatest impact of iodine deficiency on human is children's brain development retardation,mental retardation,and children's intelligence quotient in severe iodine deficiency area is reduced to 12.46 points.Iodine deficiency is a major factor in the development of mental retardation(MD)in children,resulting in thyroxin synthesis disorder and brain development disorder.Malnutrition,goiter substance,deficiency of iron intake of trace elements also affects the level of thyroid hormone and further aggravate iodine deficiency.The west of Fujian belong to severe iodine deficiency area,after years of treatment,iron are 1995 implementation of a comprehensive iodized salt,iodine deficiency has been effectively controlled,but still appear new cretinism in children.This article chooses pregnant women in the west area of Fujian province to investigate the thyroid function of pregnant women explore the nutrition level of iodine and iron in pregnant women,and the relationship with thyroid hormone level and function.Method: The 722 cases of the author,s hospital gynecology clinic were collected from the patients as the research object In May 2016-2016 in November,the T1 period of pregnancy 1-22 weeks(early)in 201 cases,T2 gestational period of 13-27 weeks(Interim)in 310 cases,T3(late 28-40 weeks of pregnancy(T3)in 211 cases.The urine,blood samples of pregnant women were collected and the urine iodine,TSH,FT3 and FT4,TPOAb,Hb,SF were detected.Among them,ammonium sulfide digestion arsenic cerium catalytic spectrophotometric method for the determination of urinary iodine,thyroid hormone were determined by chemiluminescent immunoassay,hemoglobin(Hb)portable tester for determination of serum ferritin hemoglobin,radioimmunoassay(SF),serum iron FerroZine colorimetric method(SI),and setting up a reference range of specific thyroid function during pregnancy in longyan.Application of this criterion of pregnant T1 period,T2 period,T3 period of thyroid function,the analysis of the data were analyzed by Spss17.0 statistical software.Result: The median iodine number of urine in west area of Fujian was 161.6 ug/L,and the patients with urine iodine value <150 g/L accounted for 39.61% of the total number of respondents.Median urine iodine in T1?T2?T3 is that 163ug/L?153.7ug/L and 160.8ug/L.A reference range of specific thyroid function during pregnancy in longyan is establish.The reference range of FT4 of pregnant T1 period is 12.96-23.12 pmol/L,T2 period is 10.05-20.42 pmol/L,T3 period is 9.23-18.77pmol/L.The reference range of TSH of pregnant T1 period is 0.034-4.26 mIU /L,T2 period is 0.027-5.06 mIU /L,T3 period is 0.54-3.95 mIU /L.The incidence of low FT4 in pregnant women was 1.38%,and sub hypothyroidism was 1.8%,and hypothyroidism was 0.42%.There was significant difference in the incidence of abnormal thyroid function between the Iodine deficiency group and the control group,and have statistical significance(p<0.05).The rate of pure iron deficiency(ID)was 16.58%,and iron deficiency anemia(IDA)was 13.27%.With the development of pregnancy,the hemoglobin and ferritin are falling.There was a significant difference between the iron deficiency group and the control group(p<0.05).Hypothyroidism rate and low T4 rate of pregnant women gradually increased with the deepening of anemia.The prevalence of hypothyroidism among groups was significantly different(p<0.05).Conclusion:1.Although the median of urinary iodine is at an appropriate level in west area of Fujian,there are still some pregnant women in iodine deficiency state.2.The normal range of reference to the specific thyroid function in the region of pregnancy was established.3.Iodine nutrition during pregnancy can affect thyroid function.4.Iron deficiency during pregnancy may affect thyroid function.
