| Objective:To observe the clinical effect of Nan Shaolin YijinjingTuotianshi combined with knife needle in patients with cervical spondylotic radiculopathy and to explore its mechanism.Method:Choose 60 patients with cervical spondylotic radiculopathy with cervical neck straightening were seen a doctor in QuanzhouZhengguHospital affiliated to FujianUniversity of Traditional Chinese Medicine from March 2016 to May 2017.According to the principle of random number table,the subjects was divided into control group(n=30 cases)and observation group(n=30 cases).The control group was treated with knife needle acupuncture and the observation group was given the treatment of Nan Shaolin YijinjingTuotianshi on the basis of the treatment of the control group.The clinical efficacy of the two groups was evaluated after treatment.Pain assessment was performed on the two groups before treatment,4 weeks,8 weeks and 12 weeks of treatment using the visual analog scale(VAS)scale.X-ray was used to measure the cervical curvature of the two groups before and after 12 weeks of treatment.Three-dimensional images of the necks before and after treatment were obtained by CT examination.The 3D geometric model was established by using mimics 14.01 software.The solid model was constructed by Geomagic studio 9 software,and the finite element model was formed in ANSYS11.0 to complete the entity verification.The model was loaded with a torque of 2N/cm to make the model flexion,extension,lateral bending and rotation.The equivalent stress distribution map of the C4-C7 vertebral body was observed respectively,obtain the data of each segment activity,stress the disc,articular process and joint capsule stress,the anterior longitudinal ligament tension stress,etc.The data of the cervical vertebra mechanics were analyzed before and after treatment of each group.Result:In the observation group,the total effective rate was 93.33%after 12 weeks treatment with the Nan Shaolin YijinjingTuotianshi combined with the knife needle acupuncture.It was higher than the control group,which was 83.33%(P<0.05).In terms of cure rate,the observation group was significantly better than the control group(P<0.01).The VAS scores of cervical vertebrae pain at 4 weeks,8 weeks and 12 weeks after treatment in the observation group and the control group were lower than those before treatment(P<0.05),and the scores in the observation group were significantly lower than those in the control group(P<0.05).In term of the changes of cervical curvature,the data of after 12 weeks’ treatment is larger than before the treatment in both observation group and the control group(P<0.01).The cervical curvature of the observation group was greater than that of the control group after 12 weeks of treatment(P<0.01).In the finite element model load corresponding torque so that the model can be anteflexion,extention,lateral bending,rotation activity,measuring C4-C7 vertebrae range of motion of each segment,the results of the two groups were greater than the activity levels before treatment(P<0.05),in which range of motion of the observation group was significantly larger than that of the control group.There was a statistically significant difference between the observation group and the control group(P<0.05).After treatment the tension of the anterior longitudinal ligament at extention was decreased in both observation group and control group(P<0.01),while the tension of the observation group was observed less than control group.There was a statistically significant difference between the observation group and the control group(P<0.05).The intervertebral disc stress results of each segment showed that the stress after treatment was lower than that before treatment(P<0.05).And the stress in the observation group was lower than that in the control group,there was a statistical significance in difference between the observation group and the control group(P<0.05).In term of stress of the articular process and joint capsule,the stress in the cervical spine during anteflexion,lateral bending and rotation activity was less than that before treatment(P<0.05)-And the stress in the observation group was lower than that in the control group,there was a statistical significance in difference between the observation group and the control group(P<0.05).The stress of extention was no obvious changes both in the two groups before and after the treatment,and no difference between the observation group and control group.There was no significant difference between the two groups and the changes between before and after treatment in both observation group and control group(P>0.05).Conclusion:The use of Nan Shaolin YijinjingTuotianshi combined with the knife needle acupuncture for cervical spondylotic radiculopathy can obtain better short-term clinical efficacy,relieve pain,help restore cervical curvature of patients,and improve cervical function of patients.The finite element analysis can be used for the anteflexion,extention,lateral bending and rotation activity of patients with cervical spondylotic radiculopathy to achieve physical verification,to evaluate the mechanical changes of the cervical vertebrae,and the stress distribution of the cervical vertebrae is good. |
