Objective:Through the investigation of lower urinary tract symptoms(LUTS)we aim to study the prevalence of LUTS,main symptoms and quality of life among middle and aging men.Exploring the characteristics and the risk factors of LUTS in this region,and put forward corresponding suggestions and measures on the problems found,so as to provide the the theoretical supports for further preventions and treatments of LUTS.Methods:In this study,1019 middle and aging men who met the standards in Xiacheng district of Hangzhou were selected as participants by using the method of multi-level random sampling.After obtaining their informed consent,they were investigated by questionnaire,which included general information and the International prostate symptom score(IPSS).At the same time,investigators obtained some data from the physical examination system,including prostate volume(PV)measured by abdominal ultrasound,more than 8 hours of venous blood on an empty stomach for total cholesterol(TC),fasting blood glucose(FBG),triglyceride(TG),high-density lipoprotein(HDL),and low-density lipoprotein(LDL),total prostate specific antigen(tPSA),waist circumference(WC)and body mass index(BMI).Software SPSS 19.0 was used for statistical analysis,general descriptive analysis,Kruskal-Wallis test and Chi square were used to analyze the status of lower urinary tract symptoms.The risk factors were analyzed by Chi square test and Binary logistic regression analysis.Results:1.There were 1019 participants who met the inclusion criteria in this study.The median age was 68 and the interquartile ranged from 64 to 74.The median score of IPSS was 7 and the interquartile ranged from 4 to 11;the median score of symptoms in storage period was 3 and the interquartile ranged from 2 to 6;the median score of symptoms in voilding period was 3 and the interquartile ranged from 1 to 5;the median score of postmicturition symptoms was 1,and the interquartile ranged from 1 to 2.The median score of quality of life was 3,and the interquartile ranged from 2 to 4.The prevalence rate of LUTS among middle and aging men in Xiacheng District of Hangzhou was 47.99%.In the 50?59 years old group,there were 45 people,the prevalence rate of LUTS was 17.78%.In the 60?69 years old group,there were 558 people,the prevalence rate of LUTS was 46.59%.In the 70?79 years old group,there were 246 people,the prevalence rate of LUTS was 49.19%.There were 170 people over 80 years old,and the prevalence rate of LUTS was 58.82%.The difference of LUTS severity among all age groups was statistically significant(?2=25.027,p<0.01).2.The top three symptoms of LUTS among middle and aging men in Xiacheng District of Hangzhou were nocturia,sensation of incomplete emptying and dysuria.47.50%of middle and aging men of Xiacheng District of Hangzhou needed to urinate twice or more every night.The incidence of nocturia was 33.34%in the 50-59 years old age group,46.59%in the 60-69 years old group,45.93%in the 70-79 years old and 56.47%in the over 80 years old age group.The difference of the incidence of nocturia among all age groups was statistically significant(?2=520.75,p<0.01).3.Fasting blood glucose,age,low density lipoprotein,total cholestrol,body mass index in LUTS group and non-LUTS group were significantly different(p<0.05).The Binary logistic regression analysis showed that:age>65 years old,fasting blood glucose>6.16 mmol/L,low density lipoprotein>3.36 mmol/L,body mass index>24 were risk factors of LUTS among middle and aging men(p<0.05),OR was 1.575(1.208-2.054),1.521(1.145-2.020),1.793(1.224-2.626),1.430(1.110-1.843)Conclusions:1.Lower urinary tract symptoms is a common problem among middle and aging men in Xiacheng District of Hangzhou.It has a certain degree of influence on their quality of life.2.Doctors and nurses should pay attention to the lower urinary tract symptoms of middle and aging men.Nocturia,sensation of incomplete emptying and dysuria are the three most common symptoms.The intervention of these symptoms should be strengthened in future clinical work,and the problem of nocturnal urination is particularly prominent.3.LUTS should arouse the attention of the whole society,strengthen the personalized health guidance for the middle and aging men which could easy to be understood,inform them to reduce high-sugar and high-fat food intake,take regular examinations,do appropriate exercise,maintain appropriate weight,in order to delay the occurrence and progress of LUTS,improve their quality of life.
