Theoretical Exploration And Clinical Practice Of Treating Idiopathic Pulmonary Fibrosis From The Perspective Of Blood Stasis

Objective:To find out the theoretical basis for the occurrence and development of IPF by collecting and collateral obstruction,observe and verify the effectiveness and safety of the method of Warming Yang and invigorating qi and activating blood circulation in the treatment of IPF-pulmonary impotence?Yang deficiency and blood stasis syndrome?,and provide a reference treatment plan and evidence-based basis for the clinical treatment of IPF.Method:1.Refer to the relevant literature about the research progress of Idiopathic Pulmonary Fibrosis at home and abroad,select the literature that meets the requirements with key words,and discuss the relationship between the formation and development of IPF and blood stasis and collateral obstruction from the perspective of traditional Chinese and Western medicine,etiology,pathogenesis and treatment of traditional Chinese medicine.2.72 cases were randomly divided into treatment group and control group,36cases each.Both groups were given oxygen for 16 h/d.The control group was given Wu's Fei Wei Fang orally,while the treatment group was given Tongxinluo capsule orally for 2 months.After the treatment,the lung function,blood gas analysis,chest CT,TCM symptom score and safety index of the two groups were compared and evaluated,and all data were analyzed by statistical software,P<0.05 was statistically significant.Result:1.Twenty-four literatures were screened,and it was concluded that IPF was related to blood coagulation and blood stasis in vivo.The method of activating blood circulation and dredging collaterals is effective in treating IPF patients according to TCM syndrome differentiation.2.Final completion of the study in 62 cases,31 cases in the treatment group and31 cases in the control group,there is no significant difference in age and sex ratio between the two groups,there is comparability.3.Overall efficacy:the effective rate of treatment group was 87.09%,and that of control group was 80.64%.The two groups were compared.P=0.008?<0.05?,indicating that the treatment group is superior to the control group in the overall clinical efficacy.4.Improvement of TCM symptoms:After treatment,the scores of TCM symptoms?shortness of breath,cough,phlegm,fatigue,cold and warm limbs,dullness,cyanosis of lip nail?in both groups were significantly lower than before treatment,indicating that the symptoms of both groups were improved after treatment.Statistical tests showed that besides dementia symptoms,the main symptoms and secondary symptoms of treatment group and control group were all P<0.05,indicating that the curative effect of treatment group was better than that of control group.5.Pulmonary function:FVC,FEV1 and MVV in the treatment group were improved compared with those before treatment,while only FVC and FEV1 in the control group were improved.Compared with the two groups,the treatment group had better effect on improving pulmonary function than the control group.6.Blood gas analysis:PO₂ of the two groups was significantly increased after treatment,and the effect of the treatment group was better than that of the control group.7.Chest CT:There was no significant change in HRCT before and after treatment in all cases.8.Safety indicators:There was no significant effect on vital signs,liver and kidney function in the two groups,indicating drug safety.Conclusion:1.Through literature search,combined with the characteristics of idiopathic pulmonary fibrosis,it is further confirmed that the occurrence of idiopathic pulmonary fibrosis is closely related to blood stasis and collateral obstruction,so as to find a theoretical basis for the treatment of blood stasis.2.Wu's Feiwei Decoction combined with Tongxinluo,which has the effect of warming yang,invigorating qi and activating blood circulation,is more effective than Wu's Feiwei Decoction alone in improving clinical symptoms,improving pulmonary function and improving hypoxia.It provides an effective treatment method for clinic and is worthy of clinical promotion and application.3.This prescription has strict compatibility,no liver and kidney damage and toxic side effects,and is safe and reliable.
