Background:Traditional method for diagnosis and treatment of thyroid cancer have been widely used in many countries,however,preventive central lymph node dissection(CLND),which bears several benefits for PTC patients,has been in controversy and subjected to further verification.In order to seek more individualized precision medical solutions,in this study,we investigated the specifically criterion of total removaland CLND.Methods:206 patients with confirmed PTC from January 2017 to June 2017 were recruited and grouped in the study.Patients in the experimental group underwent unilateral lobectomy and unilateral surgery.Central lymph node dissection was performed and the specimens were resected for intraoperative frozen pathology.Then,according to the frozen pathology results,postoperative complications,surgical evaluation,and the association between lymph node metastasis and clinicopathologic factors of CNO patients were evaluated.The results were compared with final surgical pathology to determine assay performance.Results:Among the patients diagnosed by ultrasound,80%and 60%were diagnosed positive by frozen and postoperative pathology,respectively.Frozen pathology and postoperative pathology detected 20%and 44%of patients with metastasis,but ultrasound can't find.In addition,comparing with diagnosis by final pathology,ultrasound and frozen pathology yielded accuracy rates of 56%and 73%in diagnosing metastasis,and 64%and 93%in determining the number of tumor nodules.The assay of intraoperative frozen pathology showed sensitivity of 46.7%and specificity of 100%.Conclusion:Frozen pathology is of great value in detection of ipsilateral CLN metastasis and provides an additional method for its diagnosis.
