Purpose:The purpose of this study was to investigate the value of magnetic resonance imaging(MRI)in reducing the positive rate of incision margin in patients with early breast cancer before breast conserving surgery.Materials:From August 2005 to June 2013,180 patients with T1-T2 grade early breast cancer undergoing breast conserving surgery were included in this study.This study was grouped according to whether or not the patients received MRI.88 patients had undergone pre-breast conserving MRI(pre-operation MRI group).92 patients underwent mammography and mammography before breast conserving operation,but only mammography and mammography before breast conserving operation.Analysis and comparison between preoperative MRI group and control group The positive rate of incision margin in breast surgery was analyzed by multivariate analysis of MRI signs which affected the positive rate of incision margin.Results:Of the 88 patients with MRI before operation,36(40.9%)underwent breast conserving operation.The results of breast color ultrasound and mammography were corrected and replaced by MRI before breast conserving operation,and the scheme of breast conserving surgery was changed reasonably.In addition,in 40 cases of invasive ductal carcinoma,the positive rates of preoperative MRI group and control group were 25%and 40%,respectively.The difference was statistically significant(P=0.02).In 16 patients with other pathological types of breast cancer,the positive rates of preoperative MRI group and control group were 28.6%and 44.4%,The difference was statistically significant(P=0.02).respectively.In 32 cases of ductal carcinoma in situ,the positive rates of the incision margin of breast conserving operation in MRI group and control group were 21.4%and 22.2%,respectively(P=0.10).Logistic multivariate analysis showed that the changes around the tumor,the size of the tumor and the distance between the tumor and the areola were the signs that affected the positive rate of the incision margin of the breast conserving operation(all P<0.05).Conclusion:Magnetic resonance imaging before breast conserving surgery can significantly improve the success rate and accuracy of breast conserving surgery,and can significantly reduce the positive rate of incision margin of breast conserving surgery.
