ObjectiveTo investigate the expression of monocyte chemoattractant protein-1(MCP-1) and IgA1 in renal biopsy tissue from IgA nephropathy.In addition,to explore the association of these two markers and their effects on the clinical feature of IgA nephropathy.MethodRenal biopsy specimens from 38 patients with a clinicopathological diagnosis of IgA nephropathy were collected.Change of pathomorphological types was according to Lee SM and Katafuchi semiquantitative scoring system.The expression of MCP-1 and IgA1 were detected with immunohisochemical and immunofluorescence staining.The expression levels were semi-quantified by visual and image analysis.The clinical data were collected from the patients.Result1.The expression of MCP-1 in IgA Nephropathy①Renal tubules epithelial cells and renal interstitial vessel wall were showed positive,Glomerulus was showed weakly positive②According to Katafuchi semiquantitative scoring system,the average scores of MCP-1 expression in renal were 0.82±0.38,4.89±0.93,7.53±0.78,11.47±0.49for no tubulointerstitial change group,mild tubulointerstitial change group,moderate and severe group.The differences were statistically significant between each two groups③MCP-1 was significantly correlated with 24 hour urinary protein excretion(p<0.05).2.The expression of IgA1 in IgA Nephropathy①IgA1 deposits respectively in mesangial region,basement membrane region,and mesangial region+basement membrane region②In Lee'sⅠlevel,mesangial region deposited was accounted for 50%, mesangial region+basement membrane region for 25%,in Lee'sⅤlevel, mesangial region deposited was accounted for 20%,mesangial region+basement membrane region for 80%③renal tubulointerstitial scores were 2.24±2.62,2.10±1.07,4.99±3.19 for IgA1 deposited in mesangial region group(A),basement membrane region(B),and mesangial region+basement membrane region group(C).The differences were statistically significant between C and A group or B group.IgA1 fluorescence intensity was not correlated with renal pathology degree. (p<0.05).3.The relationship between MCP-1 and IgA1①Scores of MCP-1 expression in renal were 5.29±2.71,4.9±3.01,8.93±3.82 for IgA1 deposited in mesangial region group(A),basement membrane region(B),and mesangial region+basement membrane region group(C).The differences were statistically significant between C and A group or B group.②IgA1 fluorescence intensity was not correlated with scores of MCP-1 expression in renal.Conclusion1.MCP-1 were differentiated expressed in tubulointerstitial tissue and were associated with the severity of tubulointerstitial damage,indicating the importance of MCP-1 in pathogenesis of tubulointerstitial damage in IgA nephropathy.2.MCP-1 was related to the clinical feature of the patient with IgAN,suggesting there association with the activity of the IgAN.3.Renal pathology damage was more severity in IgA1 deposited in mesangial region+basement membrane region group.4.MCP-1 was expressed reinforcement in IgA1 deposited in mesangial region+basement membrane region group.
