Objectives:To study the risk factors of lacunar cerebral infarction(LI),and to explore the predictive value of lipid composition level for the occurrence of LI,so as to more accurately intervene in the early stage of LI and reduce the occurrence of LI.Methods: From 2016.9 to 2018.6,all the patients admitted to our hospital(Affiliated Hospital of Hebei University of Engineering)were selected as LI group(experimental group).A total of 317 patients with non-lacunar cerebral infarction(NLI)were selected as NLI group(control group),with a total of 135 cases.Chi-square test and independent sample t-test were used to analyze the risk factors affecting LI.Bivariate logistic regression analysis was used to analyze the significant risk factors for univariate analysis.The significant risk factors for multivariate analysis of lipid composition were analyzed by ROC curve,and the predictive value of lipid composition level for the occurrence of LI was clarified.Results:Univariate analysis showed that sex,alcohol consumption,apolipoprotein A1(ApoA1)and apolipoprotein B(ApoB)were the four risk factors P > 0.05,indicating that there was no significant difference between LI group and NLI group in the above four risk factors;smoking,hypertension,diabetes,coronary heart disease,age,total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low density.Thirteen risk factors,including LDL-C,LP(a),FIB,HCY and UA,were found to be P<0.05,indicating that the above 13 risk factors were significantly different between LI and NLI groups.Multivariate analysis showed that HDL-C had no significant effect on the occurrence of LI,and smoking,hypertension,diabetes,coronary heart disease,age,TC,TG,LDL-C,LP(a),FIB,HCY and UA had significant effect on the occurrence of LI.According to the OR value of these 12 risk factors,LDL-C had significant effect on the occurrence of LI.LI has the greatest impact.The results of ROC curve analysis showed that the areas under TC,TG,LDL-C and LP(a)curves(AUC)were 0.698,0.598,0.727 and 0.676,and the critical values were 4.84 mmol/L,1.90 mmol/L,2.25 mmol/L and 237.50 mg/L,respectively.According to the area under the curve of TC,TG,LDL-C and LP(a),it is concluded that LDL-C has the greatest diagnostic value in predicting LI and should be intervened when it is more than 2.25 mmol/L.Conclusions:Smoking,hypertension,diabetes,coronary heart disease,age,TC,TG,LDL-C,LP(a),FIB,HCY and UA have significant effects on the occurrence of LI;LDL-C has the greatest influence on LI;TC?4.84 mmol/L,TG?1.90 mmol/L,LDL-C?2.25 mmol/L and LP(a)?237.50 mg/L can be used as critical indicators for predicting LI;The level of LDL-C in blood lipids has the greatest diagnostic value in predicting the occurrence of LI.
