| Objective: With the development of image technology, lacunar mfarction(LI) is gradually noted by people as a important ischemical cerebral stroke subtype. It possesses special clinical situation and pathological particularity, most researchers think it's episode has some relation with hypertension, but some think there are other major risk factors because partial patients with LI have no hypertension, so up to now there are still disputes about risk factor of LI. For that we have selected 134 inpatients to performed case-control study and employed single factor and multiple factor analysis in order to get a deeper understand about risk factor of LI.Methods: To select 134 inpatients in department of neurology of the second affiliated hospital of Dalian Medical University from 2002.3-2002.10 including 52 LI patients(22 single lesion, 30 multiple lesion), 52 cerebral thrombosis patients and 30 non-cerebral vessels disease patients(served as contrast), To evaluate possible risk factors such as sex, age, smoking history, diabetes history, hypertension history, fasting blood sugar, cholesterol,triglyceride, low density lipoprotein, high density lipoprotein, fibrinogen,plaque of carotid artery(exist or not, type, scale),stenotic degree of carotid artery for all patients. To find whether there is difference or not between solitary LI group and poly LI group, LI group and non-cerebral vessels group, LI group and cerebral thrombosis group using single factor analysis; to get the message of major risk factors resulting in LI by analyzing LI group and non-cerebral vessels disease group, LI group and cerebral thrombosis group with Logistic regression analysis.Results:一. single factor analysis(一) solitary LI and multiple LI: there are differences in hypertension history, festing blood sugar, cholesterol, triglyceride, low density lipoprotein, high density lipoprotein, fibrinogen.(二) LI group and non-cerebral vessels group: there are differences in hypertension history, fasting blood sugar, low density lipoprotein, high density lipoprotein, fibrinogen, carotid artery stenosis, plaque scale.(三) LI group and cerebral thrombosis: there are differences in age, hypertension history, diabetes history, fasting blood sugar, cholesterol, triglyceride, low density lipoprotein, fibrinogen, existence of carotid artery plaque or not, type of plaque, plaque scale, stenotic degree of carotid artery. 二. Logistic regression analysisHypertension history and high density lipoprotein are major risk factors to result in LI; Age, cholesterol and type of plaque of carotid artery(soft plaque, mixed plaque) are chief factors in relation to episode of LI or cerebral thrombosis. Conclusions:一. Episode of LI is in relation with multiple factors including hypertension history, cholesterol, fibrinogen, stenotic degree of carotid artery and plaque scale, high density lipoprotein is a protective factor for LI.Multiple Logistic regression shows hypertension history and high density lipoprotein are chief factors to affect episode of LI, of which hypertension is risk factor and high density lipoprotein is a protective factor.二. There are differences in risk factors of episode between solitary LI and multiple LI, the possibility of solitary LI is higher for person with hypertension, possibility of multiple LI is higher for person with high level of fasting blood sugar, cholesterol, triglyceride, low density lipoprotein, fibrinogen, person with low level of high density lipoprotein is easy to suffer multiple LI.三. There are differences in risk factors of episode between LI and cerebral thrombosis, person with hypertension is easy to suffer LI, however the possibility of cerebral thrombosis is higher for person with high level of age, cholesterol, triglyceride, low density lipoprotein, fibrinogen, stenotic degree of carotid artery, carotid plaque, plaque scale and low level of high density lipoprotein.Multiple regression analysis shows major factors affecting episode of LI or cerebral thrombosis are age, chol... |
