Objective:The mechanism of the slow flow of coronary artery is not clear.Studies have pointed out that there may be related with platelet,this study explores the platelet volume index,including platelet distribution width(PDW),mean platelet volume(MPV),platelet large cell ratio(P-LCR)and the relationship between coronary slow flow phenomenon,and explore the risk factors and pathogenesis of coronary slow flow phenomenon,and provide ideas for the prevention and treatment.Methods:From January 2016 to June 2017,coronary angiography was performed in 3720 patients with coronary heart disease admitted to Affiliated Hospital of Binzhou Medical College Department of Cardiology.Including slow coronary flow(SCF)in 48 cases(1.29%),Angiography normal blood flow(NCF)in 79 cases(2.12%)as control group at the same time.Age,sex,smoking,hypertension and diabetes history were collected from two groups of patients.Laboratory data WBC,PLT,PDW,MPV,P-LCR,UA and GLU were collected,and the mean TFC of branches in two groups was calculated.The risk factors of slow blood flow were analyzed and the risk was predicted.SPSS19.0 was used to carry out data statistics.P<0.05 indicated that the difference was statistically significant.Result:We found that BMI(25.7±2.2)kg/m2,23 cases of smoking(47.9%),PDW(13.0±0.8)fL,MPV(11.5±0.8)fL,P-LCR(32.8±3.1)%,UA(354.1±82.7)?mol/L in SCF group were significantly higher than BMI(24.2±2.0)kg/m2,19 cases of smoking(24.1%),PDW(11.4± 1.1)fL,MPV(1 0.4±0.8)fL,P-LCR(24.7±4.4)%,UA(282.7±59.5)?mol/L in NCF group.The risk factors were performed by multivariate Logistic regression analysis,BMI(OR=1.981,95%CI=1.006-3.900,P=0.048),PDW(OR=4.146,95%CI=1.238-13.876,P=0.021),MPV(OR=4.818,95%CI=1.259-18.445,P=0.022),P-LCR(OR=1.819,95%CI=1.289-2.565,P=0.001),UA(OR?1.015,95%CI=1.001-1.030,P=0.033)were regarded as independent risk factors for SCF.After multiple linear regression analysis,PDW(B?2.464,95%CI=0.303-4.625,P=0.026),MPV(B=3.087,95%CI=0.534-5.641,P=0.018),P-LCR(B=0.705,95%CI=0.230-1.180,P=0.004)were regarded as independent predictors of mean infarction frame counting(TFC).Conclusion:Platelet volume index PDW,MPV and P-LCR are independent risk factors for coronary slow flow phenomenon.Smoking,obesity,uric acid are related with coronary slow flow.Platelet volume index can be used as a clinical index to estimate slow coronary flow.
