Background and Objective Liver resection(LR)is one of the primary treatments for Liver tumors.Because postoperative residual liver failure can lead to acute liver failure,it is often difficult to choose between operation and residual liver function.At present,there are many studies on intraoperative intervention in liver regeneration,reported in recent years measures with combined liver and portal vein ligation of two step method of liver(associating liver partition and portal vein ligation for staged hepatectomy,ALPPS),but because of its need second operation,the risk is higher,is still controversial in clinical application.Therefore,it is of great clinical significance to develop a low-risk and effective measure to promote liver regeneration.Compared with splenectomy,splenic artery ligation can effectively reduce portal venous pressure,which requires only one operation.In addition,surgical incision selection is easier and surgical risk is greatly reduced.Spleen preservation can also preserve part of immune function.Since hepatic malignancies are often associated with cirrhosis,the exploration of liver regeneration in cirrhotic rats may be closer to clinical practice.This research through the establishment of SD rat liver cirrhosis,70% hepatectomy(Partial hepatectomy,Ph)model,the observation of splenic artery ligation of hardening of the rat liverregeneration in the process of liver regeneration enhancement factor(Augmenter of liver regeneration,ALR),across the membrane,and structure domain protein ubiquitin samples 1(Transmembrane and ubiquitin-like domain protein 1,Tmub1)expression and the effect ofTo further investigate the effect of splenic artery ligation on liver regeneration in cirrhotic rats.Materials and Methods Early experiments,the preliminary experiments,cirrhosis of the liver model(mortality 14.29%,Model success rate 70%),70% Ph model rats(no death),A total of 90 SD male rats were successfully selected,adaptive feeding 1 week(purchased from hunan slack scene of laboratory animal LTD,mass 180-220g),the use of intraperitoneal injection of 50% carbon tetrachloride solution of olive oil(0.2 ml / 100 g),the left lower abdomen,the succession of the right lower quadrant abdominal injection of a total of three times a week(every Monday,Wednesday,Friday),9 weeks after injection,cirrhosis of the liver model,random divided into 3 groups: control group:After laparotomy,the perihepatic and splenic hilum were dissociated and the abdomen was closed.Liver resection group: the perihepatic and splenic hilum were dissociated after laparotomy,and the left inner and middle lobe of the liver were resected and the abdomen was closed.Liver resection+spleen ligation group: ligate the splenic artery on the basis of liver resection group.All rats fasted for 8h before surgery and could not help water.Ten rats in each group were randomly selected to measure the relevant indicators at 24 h,48h and 72 h after the surgery,respectively.Liver regeneration was observed after anesthesia,and liver specimens were removed and abdominal aortic blood was extracted for later use.Postoperative weighing was performed and the hepatosomal ratio was calculated.The liver samples were fixed with formalin,sent to the pathology department for HE staining,and the expressions of ALR and Tmub1 in liver tissues were detected by immunohistochemistry.Enzyme-linked immunosorbent diffusion assay(ELISA)was used to detect the ALR and Tmub1 levels in serum.The differences of various indicators were analyzed by SPSS 21.0,and P<0.05 was considered to be statistically significant.ResultsThis study cirrhosis rats 70% Ph model was established successfully,Thesurvival rate of the successfully modeled rats was 100% within 1 week,1 weeks before all rats of ALT,AST before modeling different degrees higher,compared with control group,postoperative with time Ph and Ph +spleen group were significantly increased(P<0.05),Ph+spleen firm group at the same time lower than Ph group(P<0.05);Postoperative Ph,Ph+splenectomy group hepatosomal ratio increased gradually,24 h after surgery,Ph group was higher than Ph +splenectomy group(P<0.05),48 h,72h,Ph+splenectomy group were higher than Ph group(P<0.05).Compared with the sham operation group,the expression of ALR in serum and liver tissue was significantly higher in the Ph group at 24 h,48h and 72 h after surgery,and in the Ph+splenectomy group at 24 h and 48 h and 72 h after surgery than in the Ph group(P<0.05).At 24 h,the expression of ALR in serum and liver tissue was significantly higher in the Ph+splenectomy group than in the Ph group(P<0.05).At 48 h and 72 h,the expression of ALR in the Ph+splenectomy group was not significantly higher than in the Ph group(P>0.05).Compared with sham operation group,24 h after Ph and Ph+splenectomy group liver tissue Tmub1 into cytoplasm expression,48 h,72h after Ph and Ph+splenectomy group Tmub1 expression level in the serum and liver tissue were significantly increased(P<0.05),compared with the Ph group,Ph+splenectomy to plunge into groups with postoperative time expression in liver tissue decreased significantly(P<0.05)in serum of Ph and Ph+splenectomy group Tmub1 level differences at the same time there is no statistical significance(P> 0.05).Conclusions 1.Liver has obvious regeneration ability,and splenic artery ligation plays an important role in promoting liver regeneration and liver function recovery in cirrhotic rats;2.The mechanism of splenic artery ligation to promote liver regeneration may be related to the promotion of ALR expression and inhibition of Tmub1 release in liver tissues.
