Objective: To explore the difference of three methods of indirect immunofluorescence(IIF),Western blotting(IBT)and radioimmunoassay(RIA)in detecting anti-dsDNA antibodies,and to analyze the value of combined detection of three methods in the clinical diagnosis of systemic lupus erythematosus(SLE).Methods: selected 500 SLE patients diagnosed in our hospital from June2012 to December 2017,and 300 other immune patients in our hospital during the same period.The above three methods were used to detect the anti-dsDNA antibodies in two populations,and the sensitivity and specificity of the three methods and the joint detection of the three methods were compared.Results: 1.The highest detection rate of IIF,IBT and RIA was RIA(76.2%),followed by IIF(57.0%),and IBT(41.2%)was the lowest,but higher than that of the disease control group(P < 0.01).2.The sensitivity and specificity of IIF,IBT and RIA for detecting anti-dsDNA antibodies were compared.The sensitivity of IIF was 57.0% and specificity was 99.0%,The sensitivity of IBT was 41.2% and specificity was99.7%,The sensitivity of RIA was 76.2% and specificity was 92.0%,Thespecificity and positive predictive value of IBT were the highest,while the sensitivity and the negative predictive value of RIA were the highest,The consistency Kappa test showed that the consistency between IIF method and RIA method was poor(k=0.378),IBT method and RIA method was poor(k=0.251),and the consistency between IIF method and IBT method was better than the former two methods,and the Kappa value was 0.637.3.The sensitivity of IIF,IBT and RIA were 62.7%,45.7% and 81.6%respectively in SLE disease active group,while 32.9%,22.7% and 53.6% in disease stable group.The difference was significant(P < 0.01).The sensitivity of three methods in SLE disease active group was higher than that in disease stable group.4.The sensitivity and specificity of the three methods were 37.2% and99.7% respectively,and 81.2% and 91.3% respectively in parallel.The sensitivity and specificity of combined detection of anti-dsDNA antibodies by three methods were higher than that by three methods alone.Conclusion:1.Among the three detection methods in the detection of anti-dsdna antibodies in SLE group,RIA method has the highest sensitivity,IBT method has the highest specificity,and IBT method has a high coincidence rate in the detection of anti-dsdna antibody and SLE clinical diagnosis.2.Among the three methods in the detection of anti-dsdna antibodies in the active group of SLE disease,the RIA method has the highest sensitivity,the IBT method has the lowest sensitivity,the IBT method has the highest specificity,and the RIA method has the lowest specificity.The sensitivity of the three methods in the active group of SLE disease is higher than that in the stable group.3.The combined detection of anti-dsDNA antibodies by the three methods can improve the sensitivity and specificity of detection.
