ObjectiveHepatitis C antibody detected by electrochemiluminescence immunoassay(ECLIA)was subjected to HCV-RNA and recombinant immunoblot assay by using Receiver operator character:istic curve(ROC curve).RIBA double confirmation,comprehensive statistical analysis of true positive and false positive samples,to obtain a best predictive value of anti-HCV antibody?95%(Signal-to-cut-off,S/CO Value),using this threshold to limit the number of patients in clinical need to confirm the test,not only can improve the accuracy and effectiveness of our screening test,but also save hospital expenditures,providing patients with the best and reliable test results.Methods15360 patients in the hospital and outpatients of our hospital were screened by Roche anti-HCV antibody.177 samples of Roche anti-HCV antibody were screened by ECLIA method for HCV RNA detection.The samples were tested positive and clinically diagnosed.It is true positive;HCV-RNA negative specimens are supplemented and confirmed by RIBA method.After double confirmation,clinically credible true positive and false positive patients are obtained.After statistical analysis by ROC curve,anti-HCV is calculated.The optimal threshold for antibodies.ResultsA total of 10 cases with S/CO value<1 were initially screened,0 cases were positive by RIBA method,10 cases were negative(100%);54 cases were between 1.0 and 10.0,and 17 cases were positive by RIBA method,33 cases(66%)were negative,and 4 cases were uncertain.Samples with low S/CO values should be considered carefully.Comprehensive analysis should be given to give reasonable explanation.Uncertain patients should 'be followed regularly to avoid falsehood.Positive sample.The results of double confirmation by HCV-RNA and RIBA in samples with S/CO value>10.0 were positive.After the above results were statistically analyzed by ROC curve,the optimal cutoff value at this time was 8.83,and the area under the curve(AUC)was 97.1%.The results were very diagnostic.ConclusionsAfter statistical analysis and comprehensive analysis,it can be concluded that the optimal cut-off value of anti-hepatitis C antibody by Roche Cobas e 602 analyzer using electrochemical immunoluminescence method is 8.83(sensitivity=86%,specificity=100%).This will determine whether further clinical confirmation is necessary for subsequent clinical visits.This method not only reduces the number of patients who need clinical confirmation tests,but also improves the accuracy and effectiveness of our antibody screening test and reduces the rate of misdiagnosis.It also reduces the financial burden and mental burden of patients.
