Objective:Analysis1.Is paroxysmal atrial fibrillation the same risk of stroke in patients with persistent/permanent atrial fibrillation?2.Is the left atrial diameter as a predictor of stroke caused by atrial fibrillation? Methods:Collected the Patients with atrial fibrillation who were present at any time during the 12-lead ECG or Holter surveillance were hospitalized patients in the Second Hospital of JiLin University from December 2016-December 2018.Patients were divided into paroxysmal atrial fibrillation group(PAF group)and persistent atrial fibrillation group(SAF group).The differences between clinical basic characteristics and follow-up endpoint practice were compared between the two groups.Statistical analysis was performed using spss 24.0.The symbolic normal distribution quantitative data is expressed as(x ± s),and two independent sample t tests were used to compare the two groups.Qualitative data is expressed in(%),caiy chi-square test.Multivariate analysis of stroke was performed using logistic regression analysis.Kaplan-Meier method was used to analyze the survival time of different characteristics.The difference was statistically significant at P<0.05.Results:A total of 1097 patients with atrial fibrillation were included in the study,including 428(39%)patients with PAF and 669(61%)patients with SAF.Univariate analysis of baseline data for PAF and SAF groups: heart rate,congestive heart failure,chronic renal insufficiency,oral anticoagulant,aspirin,rhythm-controlled drugs,left atrial diameter,mean CHADS score,and CHA2DS2-VASc score Statistical significance(P<0.05);there was no significant difference in the history of hypertension,diabetes,dyslipidemia and previous stroke(P>0.05).Clinical endpoints: The clinical endpoints were statistically significant(P<0.05).Patients with cerebral infarction in the PAF group and the SAF group were compared with NIHSS score,DWI-ASPECTS score,ESSEN score,and hemorrhagic transformation.Selecting some statistically significant factors in the univariate analysis and binomial logistic regression analysis of stroke risk score factors: left atrial diameteris.Independent risk factors for SAF vs PAF(OR=1.100,95% CI: 1.059-1.142,P<0.001).Conclusion:1.In this cohort-based AF patient cohort,PAF had a lower risk of stroke compared with SAF,and the differences persisted after adjusting for covariates.2.Risk stratification of stroke according to type of atrial fibrillation: low risk in patients without AF;paroxysmal atrial fibrillation as intermediate risk;persistent atrial fibrillation at high risk.Patients in the PAF group and the SAF group were divided into a history of <5 years,?5 years according to the history of atrial fibrillation.The incidence rate of the PAF group <5 years was lower than that of the SAF group<5 years.The incidence of stroke was lower than that of the PAF group ?5 years.The incidence of stroke was lower than that in the SAF group ?5 years,and the difference was statistically significant(P<0.05).3.The size of the left atrial diameter is independently associated with the risk of stroke in patients with atrial fibrillation.The size of the left atrial diameter is independently associated with the occurrence of stroke in SAF patients compared with PAF patients.It is statistically significant after adjustment for baseline characteristics.
