Surgical Treatment Of Deep Branch Of Ulnar Nerve

Objective:20 cases of deep branch injury of ulnar nerve treated in our hospital since 2013 were retrospectively analyzed.The causes,clinical symptoms and surgical methods were summarized and discussed.Innovative ideas were put forward for early diagnosis and selection of appropriate surgical methods.Method:The data of 20 patients with deep branch injury of ulnar nerve admitted to our hospital from January 2013 to August 2018 were retrospectively studied.The clinical history,course,clinical symptoms,signs,value of auxiliary examinations and selection of surgical methods were summarized and analyzed.The criteria for early diagnosis,surgical indications and selection of surgical methods were discussed.The accuracy of treatment and clinical diagnosis which determined the prognosis of the disease.were judged according to the results of longterm follow-up.Result:Twenty patients in this group were followed up,including 15 males and 5 females,aged from 12 to 71 years,with an average age of 43.9 years.The course of disease ranged from 0 to 18 months,with an average of 2.6 months.The follow-up period ranged from 8.9 to 68.1 months,with an average of 37.0 months.There were 9 cases of open injury and 11 cases of closed injury.Four emergency patients underwent one-stage nerve anastomosis,one patient with nerve injury after resection of Guyon’s canal mass and one patient with explosive injury anastomosed the retracted end of the nerve after release them.The rest patients underwent neurolysis.Referring to the trial criteria of ulnar nerve function assessment of Hand Surgery Society of Chinese Medical Association,the results were excellent in 11 cases,good in 4 cases,fair in 4 cases,poor in 1 case,excellent and good rate was 75%.Conclusion:The injury of the deep branch of ulnar nerve is easy to be missed due to its complicated mechanism and hidden symptoms.Early diagnosis and surgical treatment should be emphasized in clinic.The deep branch of ulnar nerve should be repaired early to restore the intrinsic muscle function and reduce the disability rate.
