| Objective : By observing the changes of TCM clinical syndromes,nutritional indicators and inflammatory factors in patients with continuous ambulatory peritoneal dialysis(CAPD)with spleen deficiency and stasis syndrome after taking modified shenlingbaizhu powder,the effects of modified shenlingbaizhu powder on malnutrition and micro-inflammation of CAPD patients with spleen deficiency and stasis syndrome were analyzed in order to explore the effect of Jiawei Shenling Baizhu Powder on malnutrition and micro-inflammation of CAPD patients with spleen deficiency and stasis syndrome.Therapeutic effect and mechanism of powder on CAPD patients with spleen deficiency and blood stasis syndrome.Method: 64 CAPD patients with spleen deficiency carrying blood stasis syndrome,who met the inclusion criteria were selected from the department of nephrology,anhui provincial hospital of traditional Chinese medicine,and from December 2017 to December 2018.They were randomly divided into two groups,for treatment group and control group,and with 32 cases in each.Four patients were exfoliated,and 60 cases were actually completed.Patients in two groups were given routine basic treatment(peritoneal dialysis,correction of anemia,antihypertensive,etc.).Patients in the control group were given compound alpha-ketoic acid orally(4 tablets at a time,3times a day),patients in the treatment group were given compound granules of modified shenlingbaizhu powder(one dose per day in the morning and evening),and the treatment cycle was 8 weeks.The changes of TCM syndrome score and subjective comprehensive nutrition score(SGA)before and after treatment were compared between the two groups.KT/V,ALB,HGB,TRF,PA,hs-CRP,IL-6,TNF-α between the two groups were detected before and after treatment.Results:1.The total effective rate of TCM syndromes in the treatment group was 80.00%,while that in the control group was 43.33%.The treatment group improved significantly compared with the control group(P<0.05).2.There was no significant difference in the total score of TCM syndromes between the two groups before treatment(P>0.05);after treatment,the total score of TCM syndromes in the two groups decreased significantly compared with that before treatment(P<0.05);after treatment,the total score of TCM syndromes in the treatment group decreased significantly compared with that in the control group(P<0.05).3.There were no significant changes in BUN and Scr before and after treatment in both groups(P>0.05).There was no significant difference in KT/V and SGA scores between the two groups before treatment(P>0.05);after treatment,the SGA scores of the two groups decreased significantly compared with those before treatment(P<0.05);after treatment,the SGA scores of the treatment group were significantly lower than those of the control group(P<0.05);after treatment,the KT/V values of the treatment group were significantly higher than those before treatment(P<0.05);after treatment,the KT/V values of the treatment group were significantly higher than those of the control group(P<0.05).4.There was no significant difference in HGB,ALB,PA and TRF between the two groups before treatment(P>0.05);after treatment,ALB and PA in both groups were significantly higher than before treatment(P<0.05),TRF in the treatment group was significantly higher than before treatment(P<0.05),TRF in the control group had no significant change before and after treatment(P>0.05);after treatment,there was no significant difference in HGB between the two groups(P>0.05).ALB,PA and TRF in the treatment group were significantly higher than those in the control group(P<0.05).5.The two groups hs-CRP,IL-6,TNF-α were no significant differences before treatment(P>0.05).Both groups IL-6 and TNF-α after treatment decreased significantly compared with that before treatment(P<0.05).hs-CRP in the treatment group was significantly different from that before treatment,while hs-CRP in thecontrol group was not significantly different from that before treatment(P>0.05).After treatment,the levels of hs-CRP,IL-6 and TNF-α in the treatment group were significantly lower than before treatment(P<0.05),the control group decreased significantly than control group(P<0.05).Conclusion:The modified shenlingbaizhu powder improved CAPD patients with dystrophy of spleen deficiency and stasis syndrome clinical symptoms,reduced SGA score,increased KT/V,ALB,PA and TRF levels,and improved the nutritional status of patients,which may be related to the fact that the modified shenlingbaizhu powder can reduce hs-CRP,IL-6 and TNF-α levels,and inhibit the formation of microinflammation. |
