| Objective:To observe the clinical effects of combination therapy with ChaiZhiLongMu Decoction and steroid and cyclophosphamide on lupus nephritis of qi stagnation and blood stasis.Methods:66 cases of patients who were 14-70 years old with lupus nephritis of qi stagnation and blood stasis were choosed as research object of this study.The study subjects were randomly divided into two groups,33 cases in each group.The control group was treated with oral steroids and intravenous cyclophosphamide,while the treatment group was treated together with oral ChaiZhiLongMu decoction on the basis of oral steroids and intravenous cyclophosphamide.Observation the changes of urinary erythrocyte count,24 H urinary protein quantitation,serum creatinine,complement C3,antinuclear antibodies and the TCM syndrome score.12 weeks of treatment were observed,4 weeks were followed up.Then the clinical effects were compared and evaluated.Results:1.The total effective rate of clinical treatment,the control group is lower than the treatment group,which indicates that the treatment group effect better.The total effective rate of treatment group was 90.91%,and the total effective rate of control group was 75.76%.The chi square test P<0.05,the difference of treatment effect of two groups was significant.These results showed that treatment of oral ChaiZhiLongMu decoction on the basis of oral steroids and intravenous cyclophosphamide is significantly better than that of simple oral steroids and intravenous cyclophosphamide treatment.2.In improvement of TCM syndromes,the TCM syndrome score in the treatment group after treatment were less than before treatment,and the differences were statistically significant(P<0.05).The TCM syndrome score in the control group after treatment were less than before treatment,and the differences were statistically significant(P<0.05).It showed that all two groups can improve TCM syndrome.After treatment,the TCM syndrome score in the treatment group were less than the control group and the difference between the two groups was very significant(P<0.05).The total effective rate of the curative effect of TCM syndrome,the difference between the two groups was very significant(P<0.05),which showed that the treatment group was better than the control group in improving the TCM syndrome.3.24 H urinary protein quantitation,the 24 H urinary protein quantitation in the treatmentgroup after treatment were less than before treatment,and the differences were statistically significant(P<0.05).The 24 H urinary protein quantitation in the control group after treatment were less than before treatment,and the differences were statistically significant(P<0.05).It showed that all two groups therapy can reduce urinary protein.After treatment,the difference between the treatment group and the control group was significant(P<0.05),which showed that the treatment group was better than the control group in decreasing urinary protein.4.Urinary erythrocyte count,the urinary erythrocyte count in the treatment group after treatment were less than before treatment,and the differences were statistically significant(P<0.05).The urinary erythrocyte count in the control group after treatment were less than before treatment,and the differences were statistically significant(P<0.05).It showed that all two group therapy can reduce urinary erythrocyte count.There was no significant difference in the treatment effect between the two groups before and after treatment(P>0.05).It showed that there was no significant difference between the two groups in decreasing urinary erythrocyte count.5.Serum creatinine,the serum creatinine in the treatment group after treatment were less than before treatment,and the differences were statistically significant(P<0.05).It showed that the treatment group had a certain effect in reducing the serum creatinine.The serum creatinine in the control group compared with before and after treatment is no statistical difference(P?0.05).There was no significant difference in the treatment effect between the two groups before and after treatment(P>0.05).It showed that there was no significant difference between the two groups in decreasing serum creatinine.6.Complement C3,the complement C3 in the treatment group and the control group after treatment were higher than before treatment,and the differences were statistically significant(P<0.05).It showed that all two groups had a effect in improving the level of complement C3.But there was no significant difference in the treatment effect between the two groups before and after treatment(P>0.05).It showed that there was no significant curative effect between the two groups in improving the level of complement C3.7.Antinuclear antibodies,the antinuclear antibodies in the treatment group and the control group after treatment were less than before treatment,and the differences were statistically significant(P<0.05).It showed that all two groups had a effect in reducing the titer of antinuclear antibodies.But there was no significant difference in the treatment effect between the two groups before and after treatment(P>0.05).It showed that there was nosignificant difference between the two groups in reducing the titer of antinuclear antibodies.Conclusion:The results of this study showed that compared with simple oral steroids and intravenous cyclophosphamide treatment,the combination therapy with ChaiZhiLongMu Decoction and steroid and cyclophosphamide for lupus nephritis due to qi stagnation and blood stasis is better.Especially in the improvement of TCM syndromes,reduce urinary protein,the effect is better than simple western medicine treatment.So the combination therapy with ChaiZhiLongMu Decoction and steroid and cyclophosphamide for lupus nephritis due to qi stagnation and blood stasis can improve significantly the symptom,reduce urinary protein and enhance the clinical effects.The results of this study show that the combination of traditional Chinese and Western medicine has a unique advantage in the treatment of lupus nephritis. |
