Association Between Osteosarcopenic Obesity And Hypertension In The Middle-aged And The Elderly Among Minorities In Guangxi

Abstract/Summary:
ObjectiveTo investigate the prevalence of Osteosarcopenic obesity(OSO)and hypertension in middle-aged and elderly population including Jing,Maonan,Mulam,Miao and Yao in Guangxi,and to explore the distribution characteristics of OSO and hypertension in different minorities by gender,and to elucidate the relationship between OSO and hypertension.MethodsA total of 2369 people aged≄50 years were randomly selected from Jing,Maonan,Mulam,Miao and Yao in Guangxi.The demographic information and health status were collected through questionnaires,and their height,body composition,bone mineral density(BMD),and blood pressure were simultaneously measured by Martin altimeter,body composition analyzer(TANITA MC-180,Japan),ultrasonic bone mineral density tester(GE Achilles Express,USA),and electronic sphygmomanometer(OMRON HEM-1000,Japan),respectively.According to body mass index(BMI),percentage of body fat(PBF),appendicular lean mass/height~2(ALM/ht~2),grip strength(GS)and T-score,people were diagnosed with obesity,sarcopenia and osteoporosis,concurrent occurrence of those diseases was defined as OSO.All participants were classified into four subgroups in terms of the number of adverse body compositions,namely group G0(the normal),group G1(one of the number of adverse body compositions),group G2(two of the number of adverse body compositions)and group G3(OSO),with comparison regarding prevalence of hypertension among four subgroups.In addition,the association between OSO and hypertension by gender was tested using t-test,Chi-square test,analysis of variance,analysis of Pearson correlation,analysis of multinomial logistic regression and receiver operating characteristic curve.Results1.The prevalence of OSO,sarcopenia,obesity and osteoporosis in this work was 5.1%,23.2%,28.3%and 24.7%,respectively.And prevalence of OSO in Jing,Maonan,Mulam,Hmong and Yao were 7.5%,8.1%,2.4%,5.0%and1.6%,respectively,with significant difference among five minorities.Additionally,prevalence of obesity,osteoporosis and OSO of males were manifestly lower than that of females.2.The prevalence of hypertension in Jing,Maonan,Mulam,Hmong and Yao was 45.1%,50.6%,43.8%,60.6%and 45.2%,respectively.And levels of systolic blood pressure and diastolic blood pressure of males in five minorities were higher than that of females,but sex differences of blood pressure level were found only in Hmong and Yao(P<0.05).3.ALM/ht~2,GS,BMI,PBF and T-score of men or women were significantly different among five minorities at different blood pressure levels,but that of women of Yao was higher than that of the others.4.For male,systolic and diastolic blood pressure of group G3 among five minorities were the highest among G0 to G3 groups.And for female,the systolic blood pressures of group G3 among five minorities were greater than the other three groups,while manifest differences in diastolic blood pressure were not found.In addition,rates of hypertension of men or women among five minorities increased with the increase of the number of adverse body compositions.Among G0 to G3 groups,prevalence of hypertension of Hmong was higher than that of other minorities in male population,but that of Hmong was greater than that of Maonan in female population.5.The systolic and diastolic blood pressure of males and females were passively correlated to BMI and PBF among five minorities.Furthermore,diastolic blood pressure of males and females were passively associated with ALM/ht~2 and T-score in Maonan,Hmong and Mulam,while systolic blood pressure of females was negatively correlated to ALM/ht~2 and T-score in five minorities.6.The results of analysis of logistic regression revealed that risk of hypertension was passively associated with number of adverse body compositions,and high BMI and PBF were risk factors of hypertension while high GS and T-score were beneficial to keeping from hypertension.In addition,the risk of hypertension was passively correlated to ALM/ht~2 in males but negatively in females.7.The results of ROC curve showed that the best cut-off values of ALM/ht~2,GS,BMI,PBF and T-score for predicting hypertension in males were 8.15kg/m~2,19.44 kg,21.85 kg/m~2,16.55%and-1.74,respectively,and those cut-off values in female were 6.05 kg/m~2,15.84 kg,21.65 kg/m~2,27.75%and-1.90,respectively.Conclusion1.There were ethnic and gender differences in prevalence of OSO.The prevalence of OSO of Maonan was the highest among five minorities but that of Yao was the lowest in this work,and prevalence of OSO of women was higher than that of men in five minorities.2.There were ethnic and gender differences in prevalence of hypertension.The prevalence of hypertension of Hmong was the greatest among five minorities but that of Mulam was the lowest,and prevalence of hypertension of women was lower than that of men in four minorities rather than in Jing.3.The findings of this study revealed the OSO was closely associated with blood pressure,indicating that OSO might be one potential factor predicted hypertension.
