| Part1:To Investigate the Value of IVIM-DWI and DCE-MRI Quantitative Parameters in the Staging of Nasopharyngeal CarcinomaObjective To investigate the value of intravoxel incoherent motion diffusion weighted imaging(IVIM-DWI)and dynamic contrast enhanced magnetic response imaging(DCE-MRI)quantitative parameters in the staging of nasopharyngeal carcinoma.Methods This study included 61 patients with pathologically confirmed nasopharyngeal carcinoma during the period from January 2017 to April 2018.IVIM-DWI and DCE-MRI was performed in all patients.IVIM-DWI parameters of ADC,D,D*and f;DCE-MRI parameters of Ktrans,Kep,Ve and Vp were calculated.The patients were divided into low staging group(staging Ⅰ and Ⅱ)and high staging group(staging Ⅲand Ⅳ).The correlations between parameters of IVIM-DWI and DCE-MRI were analyzed between the two groups.Results ADC,D,D*,Ktrans and Kep showed a decreasing trend with the increase of clinical stage,and they were negatively correlated with the clinical stage of nasopharyngeal carcinoma.(the values of r respectively were-0.218,-0.387,-0.298,-0.327,-0.235,the values of P respectively were 0.021,0.001,0.007,0.012,0.026)Conclusion There were significant differences in the quantitative parameters of IVIM-DWI and DCE-MRI in different staging groups.The IVIM-DWI and DCE-MRI parameters have certain value in distinguishing the staging of nasopharyngeal carcinoma.Part2:To Investigate the Value of IVIM-DWI and DCE-MRI Quantitative Parameters in Early Response Prediction of Nasopharyngeal CarcinomaObjective To investigate the value of intravoxel incoherent motion diffusion weighted imaging(IVIM-DWI)and dynamic contrast enhanced magnetic response imaging(DCE-MRI)quantitative parameters in early response prediction of nasopharyngeal carcinoma.Methods During the period from January 2017 to April 2018,inducing chemotherapy(IC)and concurrent chemoradiotherapy(CRT)was carried out in 56 patients with pathologically proved nasopharyngeal carcinoma at author’s hospital’s induction.Pretreatment IVIM-DWI and DCE-MRI was performed in all patients.I VIM-DWI parameters of ADC,D,D*and f;DCE-MRI parameters of Ktrans,Kep,Ve and Vp were calculated.The patients were reexamined with MRI 6 months after the end of chemoradiotherapy.The patients were divided into nonresidual group and residual group.The correlations between pretreatment parameters of IVIM-DWI and DCE-MRI were analyzed between the two groups.Results(1)ADC and D values of nonresidual group were lower than residual group(P<0.05),and nonresidual group had higher Ktrans values than residual group(P<0.05).No significant correlations were found between any other parameters.(2)The area under ROC curve(AUC)of the baseline ADC,D and Ktrans values used to predict response 6 months after to IC and CRT were 0.664、0.759、0.711(P<0.05).Conclusion The pretxeatment IVIM-DWI and DCE-MRI parameters may be useful for prediction for early response of nasopharyngeal carcinoma patients who underwent IC and CRT and may provide a basis for individualized treatment. |
