Background and Objective Gastric cancer is one of the most common malignant tumors in the human digestive tract.The number of new cases of gastric cancer in China occupies the main part of the world,and its morbidity ranks second among all malignant tumors,and its mortality ranks first among malignant tumors of the digestive system,with higher morbidity and mortality.In 1865,French professor Armand Trousseau first reported that gastric cancer patients are prone to form venous thrombosis,and then,people have always referred to cancer patients combined with superficial thrombophlebitis as "Trousseau syndrome",and conducted extensive and in-depth discussion and research on the relationship between coagulation system and malignant tumors.The purpose of this study was to collect the clinicopathological data and coagulation index data of gastric cancer patients,to study the relationship between plasma FIB and d-dimer levels in preoperative diagnosis,tumor staging and metastasis of gastric cancer patients,and to explore the value of FIB and d-dimer levels in guiding surgery.Methods In this study,general clinical data of 105 patients with gastric cancer and 100 patients without tumor(control group)were collected strictly according to the inclusion criteria.Plasma FIB and d-dimer levels were also collected in both groups before treatment(the morning after admission).Detailed clinicopathological data of patients with gastric cancer were collected.Plasma FIB and d-dimer levels in the gastric cancer group and the control group were statistically analyzed.To study the relationship between plasma FIB or d-dimer level and gender,age,pathological type,differentiation degree,pathological stage,infiltration depth,presence or absence of lymph node metastasis,presence or absence of distant metastasis and physical condition in gastric cancer group.And statistical software was used for comparative analysis.ROC curve was used to analyze the value of two coagulation indicators in the diagnosis of gastric cancer for statistical analysis.Results Plasma FIB and d-dimer levels in patients with gastric cancer were higher than those in non-tumor patients,and the difference was statistically significant(P<0.001).Plasma FIB and d-dimer levels in patients with gastric cancer were correlated with pathological stage,depth of tumor invasion,presence or absence of lymph node metastasis,and presence or absence of distant metastasis.Patients with later TNM stage,later T stage,lymph node metastasis and distant metastasis were significantly higher than those with earlier TNM stage,earlier T stage,no lymph node metastasis and no distant metastasis,all of which had statistical significance(P < 0.05).For different degrees of differentiation,the plasma d-dimer level of patients with low differentiation was significantly higher than that of patients with medium differentiation and high differentiation(P < 0.05),while there was no significant difference in FIB level between the two groups(P > 0.05).For different physical status scores,FIB level of patients with low KPS was significantly higher than that of patients with high KPS(P < 0.05),while there was no significant difference in d-dimer level between the two groups(P > 0.05).There was no significant difference in plasma FIB and d-dimer levels among different pathological types,age and gender groups(P > 0.05).The area under the ROC curve of FIB in the diagnosis of gastric cancer was 0.837,the maximum cut-off value was2.85g/L,and the sensitivity and specificity were 85.7% and 70.0%,respectively(P <0.001).The area under the ROC curve of d-dimer in the diagnosis of gastric cancer was0.883,the maximum cut-off value was 0.525ug/ml,and the sensitivity and specificity were 78.1% and 89.0%,respectively(P < 0.001).Conclusion The plasma FIB and d-dimer levels of patients with gastric cancer were higher than those of non-tumor patients,which had guiding values for the diagnosis of gastric cancer and could improve the diagnostic sensitivity by combining the detection of tumor markers.The level of plasma FIB and d-dimer may be closely related to the TNM stage,depth of lesion invasion,presence or absence of lymph node metastasis,and presence or absence of distant metastasis of gastric cancer.The high level of plasma FIB and d-dimer may indicate that the tumor stage is late,and metastasis may have occurred,and the prognosis may be poor.Plasma FIB and d-dimer levels are of guiding value in the selection of surgical methods.
