Objective To investigate the relationship between the occurrence of meniscal root tears and the severity of knee osteoarthritis(KOA),and to evaluate the diagnostic value of MRI in the meniscal root tears.Materials and Methods A total of 59 patients with clinically diagnosed knee osteoarthritis in our hospital from June 2017 to August 2018 were retrospectively selected.All patients were on X-ray and MRI equipment routine knee scan.And perform knee arthroscopy.Patients with knee joint meniscal root tears and without roots tearing were divided into two groups.Kellgren and Lawrence(K-L)grading(KOA radiological grading)and Outerbridge grading(arthroscopy degree of cartilage damage)were compared between the two groups.The meniscal root tears and its location were diagnosed in the MRI examination results,and the results were compared with the corresponding arthroscopy results.P<0.05 for the difference was statistically significant.In the MRI examination results,the diagnosis of the occurrence of root tear and its location,then compare the results with the corresponding arthroscopy results.Results Arthroscopic diagnosis confirmed that 37 patients had a meniscal root tears in the knee joint,which was recorded as the root tear group;the other 22 patients without root tear was the control group.There were no significant differences in age,gender,height,weight,and body mass index between the two groups.According to K-L Grading System,there were 0 cases in 0 grade,9 cases in?grade,25 cases in ? grade and 3 cases in ? grade in root tear group;which were 3 cases in 0 grade,14 cases in ?grade,4 cases in ?grade and 1 case in ? grade in no root tear group;there was significant statistic difference between two groups(z=-3.847,P<0.05).The Outerbridge grading were 0 cases in 0 grade,1 case in?grade,6 cases in ?grade,12 cases in ? grade and 18 cases in ?grade in observed group;which were 1 case in 0 grade,5 cases in ? grade,2 cases in ? grade,10 case in ? grade and 4 cases in ?grade in no root tear group;there was significant statistic difference between two groups(z=-2.349,P < 0.05).Compared with arthroscopy results,the diagnostic sensitivity of MRI to root tears was 94.6%,the specificity was 90.9%,and the accuracy was 93.2%.The consistency of the two examination methods was very good(Kappa=0.95)and the difference was meaningless(x2 =2.053,P>0.05).28 cases of posterior root tears of medial meniscus,8 cases of posterior root tears of lateral meniscus,and 1 case of anterior root tears of lateral meniscus were detected in MRI,there was no significance statistical difference between the two methods(x2=1.250,P>0.05).Conclusion This study shows that the occurrence of knee joint meniscal root tears is related to the degree of KOA.The higher the degree of KOA,the higher the incidence of root tear.At the same time,MRI examination has high sensitivity and good diagnostic ability to the meniscal root tears,which can assist the clinical prevention and preoperative diagnosis better.
