| BackgroundAnal fistula is a common anorectal disease in clinic.It was generally believed that the anal gland infection caused by the plug of anal crypt gland leads to the formation of anal fistula.The incidence of anal fistula is about 1/10000,and the high incidence age range is 20~40 years old,among which male patients account for the majority,and the ratio of male to female is about 6:1.The main treatment of anal fistula is surgery,but postoperative recurrence rate is high.At present,there are many studies on the treatment of anal fistula,but few studies on the pathogenesis of anal fistula.Many foreign studies have shown that the development of anal fistula may involve histological,microbiological,molecular and host factors.Vimentin in the process of infection,can act as a signal pathways of ligands of receptor of NOD2,and NLPR3 Dectin-1 involved in immune signal transduction after pathogen invasion.vimentin can also be involved in wound healing process by influencing the fibroblast migration.Anal fistula is crissum infectious disease,experience infection and repair process.Vimentin may be involved in the process of anal fistula infection and repair,affecting its occurrence and development.However,its role in the development of anal fistula has rarely been studied.In this study,the expression level of vimentin in fistula tissue was detected by immunohistochemical method,to explore the role of vimentin expression in the development of anal fistula,and to provide a theoretical basis for the exploration of the treatment of anal fistula.ObjectiveTo study the relationship between the expression level of vimentin and thebacterial spectrum of anal fistula infection and the clinical characteristics of patients,and to explore the role of vimentin expression in the development of anal fistula.MethodsThe clinical data of 120 patients with anal fistula hospitalized in Anorectal Department of Hangzhou Third People’s Hospital from January 2017 to December2017 were retrospectively analyzed.The fistula secretions were cultured by bacteria.The expression of vimentin in fistula tissues was detected by immunohistochemical method.The relationship between bacterial spectrum and expression of vimentin was analyzed,and the main parts of the expression of vimentin and the relationship expression of vimentin and clinical characteristics of patients were analyzed.ResultsAmong 120 patients with anal fistula,Escherichia coli(63.3%),Klebsiella pneumoniae(18.3%)and Staphylococcus aureus(7.5%)were the main bacterial cultures of fistula secretions.The expression level of vimentin in E.coli group was significantly higher than that in Klebsiella pneumoniae group,Golden Grape ball group and other bacterial groups(P < 0.05).The expression level of vimentin in the high anal fistula group was lower than that in the low anal fistula group,and that in the complex anal fistula group was lower than that in the simple anal fistula group(P< 0.05).ConclusionIntestinal bacterial infection was the main cause of anal fistula infection and E.coli was the most common cause.The expression level of vimentin was related to the location of anal fistula and the degree of pathological changes.vimentin may be involved in the occurrence and development of anal fistula. |
