| Objective:Tushen antai granule is a self-designed prescription for spleen and kidney deficiency type of pre-abortion created by the tutor after years of clinical experience.In order to further understand its therapeutic mechanism and confirm its curative effect,this study was designed.1.By observing the clinical symptoms and b-mode ultrasonography of the patients with spleno-kidney deficiency type of pre-abortion with spleen and kidney deficiency before and after treatment,the disease efficacy was determined.The curative index was calculated with the integral of the syndrome of spleno-kidney deficiency with kidney deficiency,and the curative effect of the syndrome of traditional Chinese medicine was determined,so as to verify the clinical efficacy of this tushen antai granule in the treatment of spleno-kidney deficiency with kidney deficiency with prior abortion.2.Through the detection of serum TNF-α,IFN-γ,IL-6,IL-10 before and after treatment,and the calculation of TNF-α/ IL-6,IFN-γ/ IL-10 ratio changes,to explore the effect of tushen antai granule on Th1/Th2 type cytokine balance,to explore its placenta mechanism from the perspective of immunology.Methods:68 cases of threatened abortion with spleen and kidney deficiency were included.The patients were randomly divided into the treatment group(34 cases)and the control group(34 cases).The treatment group was treated with tushen antai granules,a dose of which was given once a day and a water infusion of 400 ml.The control group was treated with 150 mg progesterone capsules orally twice a day for 2 weeks.The clinical symptoms of the patients were observed before and the next day after the medication for 2weeks,and the curative effect of the disease was determined by b-mode ultrasonography,syndrome score was conducted,serum TNF-α,IFN-γ,IL-6 and IL-10 were detected,and the changes in the ratios of TNF-α/ IL-6 and IFN-γ/ IL-10 were calculated.Results : 68 cases were included in the experiment,including 4 cases in the treatment group and 4 cases in the control group.1.Curative effect of the disease: in the treatment group,21 cases were cured,7cases were effective,3 cases were ineffective,and the total effective rate was 90%;The total effective rate was 66.67%.Compared with the control group,the treatment group had better efficacy(P < 0.05,which was statistically significant).2.Syndromes curative effect: the treatment group and control group after treatment of TCM syndrome integral has the obvious drop,the treatment group and control group before and after treatment of TCM syndrome integral had significant difference(P < 0.05,with statistical significance),between the two groups after treatment of TCM syndrome integral difference is not obvious(P > 0.05),no difference between the TCM syndrome curative effect.3.Serum Th1 concentration: after treatment,serum TNF-αin pregnant women in the treatment group and the control group decreased significantly(P < 0.05),and the serum TNF-αin the treatment group decreased significantly compared with the control group(P< 0.05,statistically significant).After treatment,the serum IFN-γof pregnant women in both groups decreased significantly(P < 0.05).After treatment,the serum IFN-γ of pregnant women in both groups decreased significantly(P < 0.05,statistically significant).4.Serum Th2 concentration: after treatment,the serum il-6 level of pregnant women in the treatment group and the control group increased significantly(P < 0.05),and the serum il-6 level in the treatment group was significantly higher than that in the control group(P < 0.05,statistically significant).Serum il-10 of pregnant women in both groups increased after treatment,with significant difference(P < 0.05),and significantly increased in the treatment group compared with the control group(P < 0.05,with statistical significance).5.Serum TNF-α/IL-6 and IFN-γ/IL-10 ratios: the serum TNF-α/IL-6 ratios of pregnant women in both groups were decreased after treatment,and the difference was statistically significant(P < 0.05).The serum IFN-γ/IL-10 ratio of pregnant women in both groups decreased after treatment,and the difference was statistically significant(P <0.05).The effect of treatment group was better than that of control group(P < 0.05,statistically significant).Conclusion:1.In the treatment of threatened abortion with spleen-kidney deficiency,the curative effect and TCM syndrome effect of tushen antuo granule and progesterone capsule were significantly improved,and the curative effect of tushen antuo granule was better than that of progesterone capsule.2.Compared with progesterone capsules,tushen anbao granules can significantly reduce serum Th1(TNF-α,IFN-γ)concentration,increase serum Th2(IL-6,IL-10)concentration,reduce Th1/Th2 value,promote itsmigration to serum Th2,maintain the balance of Th1 and Th2,and reveal the tushen anbao granules in the treatment of spleen and kidney deficiency threatened abortion mechanism. |
