Objective:To investigate the status of anxiety and depression in patients with upper gastrointestinal hemorrhage due to cirrhosis,and to analyze the influencing factors of anxiety and depression,and to formulate nursing intervention program based on mindfulness decompression therapy according to the results of the study,to explore the effect of this program on anxiety and depression in patients with upper gastrointestinal hemorrhage due to Cirr.Methods:This study is divided into two stages: investigation and intervention.Investigation and study: 180 inpatients with decompensated cirrhosis were selected by convenience sampling method from January to March 2018 in a third-class A hospital in Taiyuan City,Shanxi Province.The general data questionnaire,the General Anxiety Disorder-7(GAD-7),the Patient Health Questionnaire-9(PHQ-9)and the Patient Health Questionnaire-9(PHQ-9)were used to investigate the status of anxiety and depression.The risk factors of anxiety and depression were analyzed by single factor analysis and multiple regression analysis.Intervention study: From April 2018 to January 2019,62 patients with upper gastrointestinal hemorrhage due to cirrhosis in the digestive department of a third-level hospital in Shanxi Province were selected and divided into intervention group and control group for routine nursing.The intervention group was given mindful decompression therapy-based nursing intervention on the basis of routine nursing of the control group.Before and after the intervention,62 patients met the inclusion and exclusion criteria.The general ised Anxiety Disorder-7(GAD-7)and Patient Health Questionnaire-9(PHQ-9)were used to evaluate the subjects,and the effect of the intervention program was obtained.Results:1.A total of 180 patients completed the questionnaire effectively.Among them,89 patients suffered from anxiety,the detection rate of anxiety was 49.44%,105 patients suffered from depression,and the detection rate of depression was 58.3%.2.Univariate analysis of anxiety showed that gender,family income,marital status,occupation,physical exercise,education level,hepatitis infection(duration of disease),sleep quality,leisure and entertainment had statistical significance in the detection rate of anxiety(P <0.05).The factors with statistical significance in single factor analysis(P <0.05)were included in multiple regression analysis.The results showed that gender,marital status,family income,hepatitis infection(duration of disease),sleep quality,leisure and entertainment had independent effects on the occurrence of anxiety.3.The univariate analysis of depression showed that age,type of medical insurance,occupation,physical exercise,education level,hepatitis infection(duration of disease),sleep quality,leisure and entertainment had statistical significance(P<0.05).The univariate regression analysis included the factors with statistical significance(P <0.05),indicating age.Physical exercise,education level,hepatitis infection(duration of disease),and sleep quality have independent effects on the occurrence of depression.4.Intra-group comparison shows that there are significant differences in anxiety and depression scores between intervention group and control group before and after intervention.Inter-group comparison shows that before intervention,there is no significant difference in anxiety and depression scores between intervention group and control group.After intervention,the scores of anxiety and depression in both groups have statistical significance,and the difference between intervention group and control group before and after intervention is greater than that of right.The difference of control group before and after intervention.Conclusion:1.The level of anxiety and depression in patients with upper gastrointestinal hemorrhage due to cirrhosis is relatively high,which should be paid attention to and intervened as soon as possible.2.The main influencing factors of anxiety in patients with upper gastrointestinal hemorrhage due to cirrhosis are gender,marital status,family income,hepatitis infection(duration of disease),sleep quality,leisure and entertainment.The main influencing factors of depression are age,physical exercise,education level,hepatitis infection(duration of disease),sleep quality.3.Nursing intervention program based on mindfulness decompression therapy can effectively reduce the level of anxiety and depression of patients,alleviate anxiety and depression,which is worth promoting.
