| Objective:To explore application of roche Cobas4800 HPV for cervical cytology for the posive significnace of atypical epithelial cells(atypical aquamous cells of undetermind significance,ASCUS)of patients with ASCUS in cytology in the clinical application value。Methods:Thinprep liquid based cytology testing(TCT)of the first affiliated hospital of nanchang university for further diagnosis and treatment from January 2015 to January 2018 were collected,and the results were 791 cases of ASCUS patients,with an average age of 37 ±1.57(17~81)years.Excluded cases: pregnancy,history of hysterectomy or cervical surgery,,being treated with chemotherapy or having received chemotherapy and pelvic radiotherapy.Roche Cobas4800 HPV sampler was used to take samples,and automatic sample preparation and real-time polymerase chain reaction(PCR),amplification and detection techniques were used.Provide test results for human papillomavirus(HPV)types 16,18,and 12 other high-risk HPV subtypes.All the patients were given electronic colposcopy and performed localized biopsy under the microscope for suspicious lesions.If no obvious abnormalities or unsatisfactory images were found under the microscope,cervical multi-point biopsy or cervical tube curettage were performed.All biopsy specimens were reviewed by the professional technicians from the pathology department of our hospital and the final diagnosis was made.Pathological diagnosis is the gold standard.Result:1.ASCUS is the most common abnormal result in TBS classification,with poor repeatability and diversity in diagnosis and classification in terms of histopathology.2.With the increase of cervical lesions,the infection rate of hr-hpv increased significantly,and with the increase of age,the detection rate of high-level cervicallesions also increased.3.Among the ASCUS patients,the age was 30 years old,the hr-hpv positive rate was higher,and the detection rate of high-grade cervical lesions was lower.The detection rate of high-grade intraepithelial neoplasia in hr-hpv-positive women aged over 50 years was significantly higher than that of young women.4.Among the ASCUS patients,the detection rate of high cervical lesions in the hr-hpv positive group was higher than that in the hr-hpv negative group,and the detection rate of high-grade intraepithelial neoplasia in the hpv16/18 positive group was significantly higher than that in the other hr-hpv type infection group.Conclusion:1.The histopathological diagnosis of ASCUS is diverse,so it is the severe and difficult clinical work to correctly treat patients with ASCUS and detect high-grade intraepithelial neoplasia or cervical cancer.2.The continuous and repeated infection of hr-hpv is the key factor for the occurrence and development of high-grade cervical intraepithelial neoplasia and cervical cancer.As the degree of cervical intraepithelial neoplasia increases,the infection rate of hr-hpv also increases.It is significant to apply CobasHPV detection in ASCUS patients for shunt management.3.CobasHPV detection can be used for cluster risk management in patients,but attention should be paid to individual treatment. |
