| ObjectiveTo investigate the efficacy of unicondylar knee arthroplasty on the early and middle stage of knee osteoarthritis with patellofemoral degeneration.MethodsA retrospective study was performed in 100 KOA patients following primary unilateral UKA from January 2014 to October 2018.The follow-up time was from 3 to 58 months,the mean follow-up time was 20.84-10.16 months;The patients were divided to four groups according to preoperative imaging data and intraoperative situation:27 patients with inner side patellofemoral joint degeneration placed in group A;21 patients with lateral patellofemoral joint degeneration placed in groupB;24 patients with bilateral patellofemoral joint degeneration placed in group C;28 patients with normal patellofemoral joint placed in group D;In the inner side patellofemoral joint degeneration group,22 cases were female(male 5 cases,mean age 65.33±6.95 years),in the lateral patellofemoral joint degeneration group,16 cases were female(male 5 cases,mean age 66.81 ±7.67 years),in bilateral patellofemoral joint degeneration group 17 cases were female(male 7 cases,mean age 68.08±6.28 years),in Normal patellofemoral joint group:21 females,7 males with an average age of 66.79 ±6.28 years;The results of statistical analysis showed that there was significant difference in the short and medium term curative effect of unicondylar knee arthroplasty in the treatment of knee osteoarthritis complicated with degeneration of patellofemoral joint by collecting baseline data,evaluation index and imaging data of patients who have been followed up for more than one month.ResultsAmong the four groups,the characteristics and preoperative HSS score,VAS score of patients were not statistically significantly different.The postoperative HSS score was 61.33±12.08、59.33 ± 11.00、61.75 ± 11.73、60.86±11.39,the HSS score 3 months after operation was 86.81±7.81、8.14 ± 6.27、86.96 ± 7.42、87.18 ± 5.89,the HSS score 6 months after operation was 91.25±5.31、90.1±4.66、90.62±6.89、90.14±4.35,the HSS score 1 year after operation was 1.33±0.96、1.19±0.98、1.21±1.32、1.04±0.84,the difference values were 4.51±1.19、4.43±1.12、4.67 ± 1.24、4.93 ±1.12.Hospital stay was 10.59±2.00 days,11.28±2.89 days,9.91±1.86 days,11.14±3.13 days.The difference was statistically significant compared with preoperative HSS score and VAS score(P<0.01).Deep venous thrombosis and pulmonary embolism were not found in the four groups after operation.Group A(2 cases),group B(2 cases),group C(5 cases),group D(4 cases)had incision complications,but there was no significant difference among the four groups(P>0.05). |
