Objective To investigate the relationship between the concentration of hydrogen sulfide?H2S?in knee joint fluid of patients with knee osteoarthritis and the knee joint function score?HSS score and VAS score?and the imaging X-ray grading of the knee joint by detecting the concentration of H2S in knee joint fluid of patients with knee osteoarthritis and that in volunteers.This experiment would provide a clinical basis for the further study of H2S.Methods We Randomly selected 90 patients of the West Coast Hospital of the Affiliated Hospital of Qingdao University from January 2018 to June 2018,who are planning to operate because of knee osteoarthritis or conservative treatment of intra-articular injection.The 90 patients was referred as the knee osteoarthritis group.In the osteoarthritis group,the inclusion criteria were strictly in accordance with the guidelines for the treatment of knee osteoarthritis in the annual meeting of the American College of Orthopaedic Surgeons.Another 10 normal volunteers was served as the control group.A small amount of joint effusion was taken from the knee joint cavity of the patient and volunteers by surgical and outpatient procedures,and then injected into a sterile negative pressure tube to seal and mark the information.The H2S concentration of the two groups was determined by enzyme-linked immunosorbent assay.According to the clinical manifestations,patients and volunteers filled in the knee joint HSS score and VAS score with corresponding information.Each of the participants in the trial routinely performed a weight-bearing X-ray examination of the knee joint.A senior orthopaedic physician and a senior imaging physician used the Kellgren-Lawrence grading standard to analyze the imaging X-ray grading of the knee joint.Results?1?The demographics data of the participants between two groups,including gender,age,height and weight,were statistically analyzed.The difference was not statistically significant?P>0.05?;?2?The content of H2S of knee joint fluid in the osteoarthritis group was?23.48±6.28??mol/L,and that in the normal control group was?43.23±2.60??mol/L.The content of H2S in the osteoarthritis group was obviously lower than the control group.The two groups?t=18.70,P<0.05?were statistically different;The HSS score of the knee joint in the knee osteoarthritis group was?53.59±12.19?,and the HSS score in the normal control group was?90.90±2.47?.The HSS score in the knee osteoarthritis group was significantly lower than that in the normal group.The two groups?t=24.81,P<0.05?were statistically different;The VAS score of the knee joint in the knee osteoarthritis group was?5.40±2.45?,and the VAS score in the normal control group was?1.03±0.32?.The VAS score in the knee osteoarthritis group was significantly higher than that in the normal group.The two groups?t=-15.72,P<0.05?were statistically different;?3?The content of H2S of knee joint fluid in the osteoarthritis group was positively correlated with the knee joint HSS score by pearson correlation analysis?r=0.693,P<0.05?,which was negative correlated with the knee joint VAS score by pearson correlation analysis?r=-0.767,P<0.05?.There was a negative correlation between the content of H2S of knee joint fluid in the osteoarthritis group and the imaging X-ray grading of the knee joint using spearman correlation analysis?r=-0.788,P<0.05?.Conclusion?1?It was found that the knee joint fluid contains H2S gas.Compared with the normal control,the knee osteoarthritis group had the lower H2S concentration of the knee joint fluid and HSS score of the knee joint,and the higher VAS score of the knee joint.?2?In patients with knee osteoarthritis,the lower the concentration of H2S in the knee joint fluid,the lower the HSS score of the knee joint,the higher the VAS score of the knee joint,the higher the imaging X-ray grading of the knee joint and the heavier the corresponding clinical symptoms,which provides a possible and clinical basis for further exploration of the role of H2S in the pathophysiology of knee osteoarthritis and new prevention and treatment methods.
