Abstract/Summary:
Purpose:Objective To observe and compare the diagnosis of gastric cavity filling with ultrasound and gastroscopy for the diagnosis of the same disease,and to explore the clinical application value of gastric cavity liquid filling ultrasound in the diagnosis of gastric and duodenal diseases.Methods:From November 2016 to January 1919,293 outpatients and inpatients and physical health examinations were treated in our hospital for symptoms such as upper abdominal discomfort.The patients completed the gastroscopy and gastric cavity liquid filling ultrasound examination at the same time.Comparative analysis of the detection of common gastroduodenal diseases.For 156 patients with suspected gastric cancer and precancerous diseases,the two tests were compared with the gold standard of pathological results,and the diagnostic coincidence rate was calculated.The sensitivity,specificity,positive predictive value and negative predictive value of the two diseases were calculated..For pathologically diagnosed benign and malignant diseases,statistical analysis of blood flow grading diagnosed by color Doppler flow imaging can effectively distinguish the degree of benign and malignant diseases.Result:1.Gastric fluid filling ultrasound examination of 293 people with gastroesophageal reflux,chronic superficial gastritis,chronic atrophic gastritis,gastric polyps,gastric ulcer,stomach cancer,duodenum-gastric reflux,duodenal ulcer The positive rate of disease detection for duodenal germination was 19.11%,92.83%,5.46%,12.68%,11.95%,7.50%,21.50%,5.12%,and 15.36%,respectively,while the gastroscope had the same disease for the above population.The positive rates of the tests were 15.02%,96.23%,14.67%,16.38%,13.99%,8.19%,17.04%,9.21%,and 19.80%,respectively.Comparing the positive rate of each disease,statistical analysis showed that the positive rate of atrophic gastritis and duodenal inflammation was p<0.05,the difference was statistically significant.The positive rate of other diseases was p>0.05,no statistical difference;2.Suspected gastric cancer and precancerous disease patients with biopsy pathological examination as the gold standard,gastric cavity fluid filling ultrasound examination of atrophic gastritis diagnosis of atrophic gastritis sensitivity,specificity,positive positive predictive value,negative predictive value were 31.57%,96.61%,75.00%,81.43%;diagnosis of gastric polyps were 71.74%,96.36%,89.19%,89.08%;diagnosis of gastric ulcer were 72.09%,96.46%,88.57%,90.08%;diagnosis of gastric cancer were 86.36%,97.96%,86.36%,97.96%.The sensitivity,specificity,positive positive predictive value and negative predictive value of atrophic gastritis diagnosed by gastroscopy were 100%,95.76%,88.37%,and 100%,respectively;the diagnosis of gastric polyps were 100.00%,98.18%,95.83%,and 100.00%,respectively;Gastric ulcers were 93.02%,99.11%,97.56%,and 97.39%,respectively;the diagnosed gastric cancers were 90.91%,97.01%,83.33%,and 98.48%,respectively;3.The diagnosis of gastric filling with ultrasound was consistent with pathological diagnosis in 95 cases.The diagnostic coincidence rate was 60.89%,and the gastroscopic diagnosis was consistent with pathological diagnosis in 144 cases.The diagnostic coincidence rate was 92.30%,which was consistent with the diagnosis of gastric cavity fluid filling ultrasound and gastroscope.Rate,using statistical analysis,p<0.05,the difference was statistically significant;4.5 cases of early gastric cancer confirmed by operation and pathology,17 cases of advanced gastric cancer;3 cases of early gastric cancer and pathological examination were detected by liquid filling ultrasound in gastric cavity,and 14 cases of advanced gastric cancer were consistent with pathological examination.Ultrasound examination of gastric cavity fluid filling has a diagnostic sensitivity of 60% for early gastric cancer and a specificity of 82.35%.The diagnostic sensitivity for advanced gastric cancer is 82.35% and specificity is 60%.5.The blood flow signals of the benign lesions of the disease blood flow combined with CDFI are less graded from 0 to I,and the blood flow signals of malignant lesions are mostly grade II to III.Precancerous disease refers to benign lesions that may develop into cancer.The blood flow signals of 95 patients with gastric cavity fluid filling and ultrasonography were analyzed,p<0.05,the difference was statistically significant,precancerous disease and cancerous Different blood flow signals.Conclusion:1.For reflux condition,the gastric cavity liquid filling ultrasound can observe the reflux state of the auxiliary agent in real time,and can avoid the false negative result under the gastroscopy;2.Ultrasound examination of gastric cavity fluid filling can observe lesion infiltration,lymph node and abdominal organ metastasis,and provide more reliable guidance for patients’ perioperative methods;3.Gastric fluid filling ultrasound examination combined with CDFI can distinguish gastric benign and malignant lesions more effectively;4.For other diseases with non-reflux conditions,the gastric cavity fluid filling ultrasound examination is inferior to gastroscopy,and still needs to be jointly diagnosed by gastroscope as an effective supplement for gastroscope.
